Submit the PRAPARE® Assessment for Review

When a patient's PRAPARE® assessment needs to be reviewed, you can submit the PRAPARE® form to a recipient within the same practice as a task.

1. Open the PRAPARE® Assessment Tool template.
2. Document the patient’s social determinants.
3. In the Enabling services provided section, do one of the following:
   • To include services for review, select Yes, select a service, and then select .
   • To exclude all services from review, select No.
     Note: The list of available services is configurable using the Enabling Services custom DB picklist. For more information on how to customize the DB picklist, go to NextGen Healthcare Success Community, and download the Custom DB Picklists System Template for NextGen® Adaptive Content Engine.
4. To document recommended local resources and support information for the patient's psychosocial needs, select Community Resources.
5. To add a referral, select Referrals.
6. Select Send Task.
   The Select Task Recipients window opens.
7. From the Available Users/Workgroups section, select task recipients.
   

8. Set a priority, select Add, and then select OK.
   A PRAPARE® assessment review task is submitted and appears in the reviewer's Inbox.
