Open the Assessments Tab

You can open the assessment tab using Add or Update Assessment template. Open NextGen® Enterprise EHR. Open the SOAP template, then in the Assessment/Plan section, select Add Assessments.The Add or Update Assessment template set opens. Optional: From the Intake or SOAP template, select the My Plan, and then select Assesments.The Add or Update Assessment template set opens. Documenting Assessment Details

You can add or update a patient's assessment details in the Assessments tab. You can update the ICD-10 coding and 5010 standards using the patient's assessment. You can select the assessment details in the grid and submit the first 12 diagnoses for claim processing. You can add or update an assessment in the following sections: Billing Diagnosis History Clinical Problems My Favourites The Today’s Assessments grid on the Assessments tab contains the following details: First column displays the priority, considering that you can submit only the first 12 row items for claims. Second column displays the diagnosis description, diagnosis code, and if entered, the status, side, and site details. Third column displays the differential diagnoses. Note: If an HCC column appears in the grid, the practice administrator must turn on the Hierarchical Condition Category (HCC) coding functionality in the Problems module. Sorting Assessments

You can update the order of the assessments for a patient in the Reorder Assessments template. The first 12 line items are available for billing, providers may want to ensure that certain assessments are included in the upper 12 rows of the list. Open NextGen® Enterprise EHR. Open a patient's record, and then open an encounter. Open the Intake template, select the specialty as Family Practice and the visit type as Office Visit, and then select My Plan. Select the Assessments tab and then select Sort. Reorder Assessments window displaying the existing sort order and new sort order of assessments The Reorder Assessments template opens. In the Existing Sort Order grid, select the row you want to be the first in the list. That selection becomes number 1 in the New Sort Order grid. Continue to select the available assessment rows until you have them in the required order. Optional: If you select a row in error, select Undo Last. To start over, select Undo All. Grids in Add or Update Assessment Template

You can add or update the assessment template. The Add or Update Assessment template includes the following grids. Grid Description Today’s Concerns/Reasons for Visit Displays the concern or reason of visit. Billing Diagnosis History Displays a list of active diagnoses for this particular patient. Clinical Problems Displays a list of all active problems that have been entered into the Problems module for this patient. Remove an Assessment from Add or Update Assessment Template

If you remove an assessment from the Add or Update Assessment template, you must add an assessment to carry over to other templates that share the same working assessment. Open NextGen® Enterprise EHR. On the Add or Update Assessment template, select the Assessments tab. In the Today's Assessment grid, select an assessment. Select Remove. Select Save & Close.
