Patient's Reason for Visit

You can document a patient's reasons for visit. In the Reason for Visit panel, you can document the History of Present Illness (HPI), problems, and diagnosis history as the reason for a patient visit. When you select a reason for visit that is already selected in the current encounter, an alert appears indicating that the reason for visit is already selected. You can either enter the duplicate reason for visit, or remove the duplicate. For a reason for visit that has separate listings for different specialties, such as abrasion, if you select two HPIs from different specialties for the same reason, an alert appears. You can select up to fourteen HPIs simultaneously. Additionally, you can select the Do not launch HPI checkbox and can select any HPI to open the corresponding HPI template. Note: You can configure sub menu items for reason for visit on the Framework Content system template for the specialty. Review Reasons for a Patient's Visit

You can review and update reasons for a patient's visit during the provider workflow. Open NextGen® Enterprise EHR. Open the SOAP, and then expand the Reason for Visit panel. Review and update the patient summary in the Introduction field. Review and update reasons for visit, diagnostic studies, and chronic conditions. To add any additional information, select Comments. Review Intake Comments on the SOAP Template Review comments entered during the intake if the Intake Comments link on the SOAP template indicates that content is present. Edit a Reason for Visit on the SOAP Template You can edit existing reasons for visits on the SOAP template. Add and Review Comments for the History of Present Illness You can review and enter additional comments for a patient's history of present illness (HPI). Remove a Studies Reviewed on SOAP Template You can remove the historical diagnostic studies that are added to the history of patient illness in the Studies Reviewed section on the SOAP template.

Copy Reasons for Visit from Previous Encounters

You can copy reasons for a patient's visit from previous encounters to the current encounter instead of manually adding them. Open NextGen® Enterprise EHR. Open the Intake template, and then expand the Reason for Visit panel. Select HPI History. The Historical Information - HPI window opens. Note: The records are displayed for the default duration configured in the Copy Previous Visit Information section of the Practice Configuration template. To filter the records in the grid, enter the search criteria in Filter by, and then select Search. Note: If the records are displayed from any sensitive encounter, an asterisk (*) appears on the record in the Sensitive column to help you decide whether to copy such records and update the current encounter as sensitive. Add Reasons for Visits

You can add HPIs as the reasons for a patient's visit. Open NextGen® Enterprise EHR. On the Intake template, in the Reason for Visit panel, select Do not launch HPI. Note: You can configure the Do not launch HPI checkbox on the Practice Configuration template. For more information, go to NextGen Healthcare Success Community, and download the Practice Configuration Template User Guide for NextGen® Adaptive Content Engine. To select the appropriate complaint, do one of the following: Select a link from the left pane in the Reason for Visit panel. Select Additional / Manage to choose maximum six reasons from the resulting template. Note: If the selected reason is a duplicate, a message appears highlighting the duplication. Document the Reason for Visit

The Reason for Visit panel is where the reason for visit, as described by the patient, is documented, along with an initial History of Present Illness (HPI). The provider should review reasons for visit, update if necessary, and complete HPI information. Reason for Visit Open NextGen® Enterprise EHR. If the Introduction field contains text, review the content and make any necessary changes.The introduction appears as the first sentence of the visit note, and it can be edited for the individual patient. A typical introduction might contain the patient's age, gender, and the reason for visit. If the introduction field is blank, compose an introduction for the patient. If checked, click to clear the Do not Launch HPI checkbox. In the Reason for Visit data grid, click one of the reasons for visit entered previously to open the HPI template to add additional details. The reason for visit added by the intake staff on the Intake template appears on the SOAP template. The Introduction field is an editable field that displays a brief summary of the patient's reason for visit. Open NextGen® Enterprise EHR. On the SOAP template, in the Reason for Visit panel, do one of the following: Select one of the reasons for visit entered previously. Select a complaint from the reasons for visits in the left pane. The associated HPI template opens. Note: If you select reason(s) for visit using the Additional / Manage link, then the reasons appear in the Chief Complaint column in the data grid. You must select the reasons listed in the Chief Complaint column in the data grid to open the associated HPI template. Reason for Visit panel On the associated HPI template, enter the appropriate data. Enter any comments. Select Save & Close.
