Document Reasons for a Patient's Visit

You can document up to six reasons for a patient's visit during intake. Open NextGen® Enterprise EHR. Open the Intake template, and then expand the Reason for Visit panel. To add reasons for the patient's visit without opening another template, select Do not launch HPI, and then do one of the following: Select the reason for visit from the left navigation bar. To add a customized reason, enter the reason in Other, and then select Add. Select Additional / Manage, select appropriate reasons from the Reason for Visit window, and then select Save and Close. Note: You can view all the available history of present illnesses (HPIs) by selecting the All hpis checkbox in the Reason for Visit window. Document Chronic Conditions

You can document patient's chronic condition details in the Reason for Visit panel. Open NextGen® Enterprise EHR. In the Reason for Visit panel, select the Chronic Conditions button.The Chronic Conditions template opens. Chronic Conditions The provider uses the Overall chronic conditions HPI field to enter notes regarding all chronic conditions of a patient. Select the Flowsheet link to open the Flowsheets template. Select the Immunizations link to add immunizations details. Select the My Phrases link to open My Phrases functionality. Select Add Problem to search for and add a problem. The added problems appear in the Problem Description list on the left. The Problem Description list expands to a full screen list if the user selects the arrows. Select a problem in the list to fill the Problem description, ICD description, and ICD code fields. You can edit the Problem description field for a patient and problem, if required. Patient's Reason for Visit

You can document a patient's reasons for visit. In the Reason for Visit panel, you can document the History of Present Illness (HPI), problems, and diagnosis history as the reason for a patient visit. When you select a reason for visit that is already selected in the current encounter, an alert appears indicating that the reason for visit is already selected. You can either enter the duplicate reason for visit, or remove the duplicate. For a reason for visit that has separate listings for different specialties, such as abrasion, if you select two HPIs from different specialties for the same reason, an alert appears. You can select up to fourteen HPIs simultaneously. Additionally, you can select the Do not launch HPI checkbox and can select any HPI to open the corresponding HPI template. Note: You can configure sub menu items for reason for visit on the Framework Content system template for the specialty. The Provider Workflow

You can review the patient's medical summary details on the Home Page template, and then open the SOAP and Finalize templates. The gastroenterologist provider workflow involves the following: Reviewing and updating the documented by the intake staff during the patient encounter and providing any additional information. Documenting problems and various physical examination findings. Documenting performed procedures. Creating a treatment plan. Documenting Evaluation and Management (E&M) codes. The E&M coding and sign-off functionalities are available on the Finalize template. Review of SOAP Details by Provider

You can review and modify the patient’s reasons for visit, review of systems, and vital signs documented by the intake staff on the SOAP template. You can also review the patient's visit type, alerts, and risk indicators. In addition, you can document the physical examination details, assessment plans, and provider communications. You can review and modify the following details on the SOAP template. General information Reasons for visit History Review of systems Vital signs Physical examination Assessment and plan Narrative notes You can select the Record contains substance use disorder information checkbox on the SOAP template header to include the long or short form of disclosure for Substance Abuse and Mental Health Services Administration (SAMHSA) 42 CFR Part 2 provisions in the generated documents and reports. Associate Medications

The Associate Medications template allows you to view and associate an active medication with up to three active diagnoses. The Associate Medications template allows you to view and associate an active medication with up to three active diagnoses. Instead of navigating back and forth from the Medications module to the Chronic Conditions template to identify and associate all of the active medications for the problem, you can use only the Associate Medications template to associate medications and diagnoses. The Diagnosis, Medication, ICD Code, Dx1, Dx2, and Dx3 fields are filled only if you have selected a problem and the associated medication in the Chronic Conditions template. The Active medications grid displays all the active medications of the patient and the Active diagnoses grid of the patient displays all the active diagnoses associated with the patient.
