You can remove a reason for visit if it is not applicable for the current encounter.
 
  - Open NextGen® Enterprise EHR.
 
  - Open the Intake or SOAP template, and then expand the Reason for Visit section.
 
  - Select Select Reason(s) for Visit.
   
    The Select Reason for Visit window opens.
   
 
  - Select Clear for the applicable reason for visit.
 
  - Select Save and Close.
