Remove a Reason for Visit

You can remove a reason for visit if it is not applicable for the current encounter. Open NextGen® Enterprise EHR. Open the Intake or SOAP template, and then expand the Reason for Visit section. Select Select Reason(s) for Visit.The Select Reason for Visit window opens. Select Clear for the applicable reason for visit. Select Save and Close. Document Reasons for a Patient's Visit

You can document up to six reasons for a patient's visit during intake. Open NextGen® Enterprise EHR. Open the Intake template, and then expand the Reason for Visit panel. To add reasons for the patient's visit without opening another template, select Do not launch HPI, and then do one of the following: Select the reason for visit from the left navigation bar. To add a customized reason, enter the reason in Other, and then select Add. Select Additional / Manage, select appropriate reasons from the Reason for Visit window, and then select Save and Close. Note: You can view all the available history of present illnesses (HPIs) by selecting the All hpis checkbox in the Reason for Visit window. To add reasons for the patient's visit with additional details by using another template, clear the Do not launch HPI checkbox, select the necessary reasons, and then select Save & Close . Edit a Reason for Visit on the SOAP Template

You can edit existing reasons for visits on the SOAP template. After the intake staff documents the patient's reasons for visit, you can edit the reasons for visit on the SOAP template to document additional details. Open NextGen® Enterprise EHR. On the SOAP template, expand the Reason for Visit panel. To edit a reason for visit, do the following: Select the HPI that you want to edit in the Reason for Visit grid. The corresponding HPI template opens. Edit the details in the HPI template. Select Save & Close. Copy Reasons for Visit from Previous Encounters

You can copy reasons for a patient's visit from previous encounters to the current encounter instead of manually adding them. Open NextGen® Enterprise EHR. Open the Intake template, and then expand the Reason for Visit panel. Select HPI History. The Historical Information - HPI window opens. Note: The records are displayed for the default duration configured in the Copy Previous Visit Information section of the Practice Configuration template. To filter the records in the grid, enter the search criteria in Filter by, and then select Search. Remove Reviewed Diagnostic Studies

You can remove the diagnostic studies from the Reason for Visit panel. Open NextGen® Enterprise EHR. Open the Intake template. In the Reason for Visit panel, do one of the following: Select the Studies Reviewed record from the grid, and then select Remove. Select Diagnostics to open the Diagnostic History Entry template, then select a diagnostic study from the grid, and select Remove from HPI Grid. Note: To turn on the Remove from HPI Grid button, you must select a study that is previously added to the grid on the Diagnostic History Entry template. The Studies Reviewed row no longer appears in the grid of the Reason for Visit panel. Note: Removing the record from the Reason for Visit panel does not remove it from the grid on the Diagnostic History Entry template or the History Summary panel. Remove a Studies Reviewed on SOAP Template

You can remove the historical diagnostic studies that are added to the history of patient illness in the Studies Reviewed section on the SOAP template. Open NextGen® Enterprise EHR. Open the SOAP template. In the Reason for Visit panel, select the historical diagnostic study item with the Studies Reviewed label in the Reason for Visit grid. Select Remove.
