Document Female Urogenital

The HPI-Female Urogenital template enables you to document a female patient's urogenital concerns and symptoms. Open the Intake template. In the Reason for Visit panel, select Gyn problems in the left pane. Note: If Gyn problems reason is not listed in the left pane, you can select Additional / Manage, and then select Gyn problems to add the reason in the Reason for Visit grid. Document a Pap and HPV Screening

The Abnormal Pap - HPI template enables you to document, view, and track a patient’s Pap and HPV screenings and the pertinent treatments. Open the Intake template. Expand the Reason for Visit panel, and then select the abnormal pap smear link. The Abnormal Pap – HPI template opens. Abnormal Pap – HPI template Under the Pap Screenings panel, select the applicable details for Onset, Location, and Status. To quickly review the prior visit summary, select Prior Visit Summary, review the details, and close the template. In the Pap and HPV Screening section, select the Screening date field. The Historical Test – Quick Entry template opens. Select a Pap test and the date of the test, and then select OK. In Pap Results and HPV Detail, select the applicable Pap and HPV results. Select Update. The information is added to the Pap Result, HPV Detail and Treatment grid. To add the documented Pap and HPV screening to the HPI summary, select the record in the Pap Result, HPV Detail and Treatment grid

Document the Details for an Annual Well Woman Visit

Open the Intake template. In the Reason for Visit panel, select annual exam in the left pane. Note: If annual exam reason is not listed in the left pane, you can select Additional / Manage, and then select Annual exam to add the reason in the Reason for Visit grid. On the Well women - Subjective template, enter the birth control method and the reason for no birth control at intake, if required. Document the required details in the following sections on the template: Menses Breast Diagnostic History Postmenopausal Menopausal symptoms Additional Symptoms Nutrition Social History Exercise/Activity Tobacco/Vaping Alcohol Select OK to close the template. Document Reasons for a Patient's Visit

You can document up to six reasons for a patient's visit during intake. Open NextGen® Enterprise EHR. Open the Intake template, and then expand the Reason for Visit panel. To add reasons for the patient's visit without opening another template, select Do not launch HPI, and then do one of the following: Select the reason for visit from the left navigation bar. To add a customized reason, enter the reason in Other, and then select Add. Select Additional / Manage, select appropriate reasons from the Reason for Visit window, and then select Save and Close. Note: You can view all the available history of present illnesses (HPIs) by selecting the All hpis checkbox in the Reason for Visit window. To add reasons for the patient's visit with additional details by using another template, clear the Do not launch HPI checkbox, select the necessary reasons, and then select Save & Close . Additional Reason for Visit Panel Features

Additional features on the Reason for Visit panel enable you to adjust how you document chief complaints and HPI information, indicate follow-up visits, review the patient's HPI history, and remove studies reviewed from the grid. Feature Description Do not launch HPI Enables you to select one or more chief complaints/reasons for visit from the list without opening the HPI template To open the HPI template after selecting reasons for visit, click the row of one of the chief complaints listed in the grid. Additional/Manage Located in the left pane and opens the Reason for Visit template, where you can indicate a follow-up visit or you can type a chief complaint. Reasons for Visit of Patients

You can document a patient's reasons for visit. On the Reason for Visit panel, you can document the common reasons for a patient visit. You can view the configurable specialty-specific reasons for visit list on the left pane. When you select a reason for visit that is already selected in the current encounter, a message appears highlighting the duplication. You can retain or remove the duplicate reason for visit. You can select up to 14 History of Present Illness (HPIs) simultaneously. Additionally, you can select the Do not launch HPI checkbox and any HPI to open the corresponding HPI template. Note: You can configure sub menu items for the reason for visit on the Framework Content system template for the specialty. Based on the configuration, on the applicable template, the sub menu items appear in the Reason for Visit panel on the left pane under the respective HPI.
