Patient's Reason for Visit

You can document a patient's reasons for visit. In the Reason for Visit panel, you can document the History of Present Illness (HPI), problems, and diagnosis history as the reason for a patient visit. When you select a reason for visit that is already selected in the current encounter, an alert appears indicating that the reason for visit is already selected. You can either enter the duplicate reason for visit, or remove the duplicate. For a reason for visit that has separate listings for different specialties, such as abrasion, if you select two HPIs from different specialties for the same reason, an alert appears. You can select up to fourteen HPIs simultaneously. Additionally, you can select the Do not launch HPI checkbox and can select any HPI to open the corresponding HPI template. Note: You can configure sub menu items for reason for visit on the Framework Content system template for the specialty. Based on the configuration, on the applicable template, the sub menu items appear

Recording Patient Details in the Intake Workflow

During check-in, the intake staff can use the Intake template to document a patient details, such as the chief complaint, vital signs, medications, allergies, standing orders, and various other findings. Panel Description General To document the patient's general information. Reason for Visit To document the reasons for the patient's visit. History Summary To document the patient's past medical history, social, medical or surgical, diagnostic study, and family histories. Note: The History Summary section displays the patient history documented on the Histories template . Vital Signs To document and manage the patient's vital signs. Medications To view the patient's current medications and medication history. You can also enter additional prescriptions and reconcile the listed medications with the actual active medications. Additional Reason for Visit Panel Features

Additional features on the Reason for Visit panel enable you to adjust how you document chief complaints and HPI information, indicate follow-up visits, review the patient's HPI history, and remove studies reviewed from the grid. Feature Description Do not launch HPI Enables you to select one or more chief complaints/reasons for visit from the list without opening the HPI template To open the HPI template after selecting reasons for visit, click the row of one of the chief complaints listed in the grid. Additional/Manage Located in the left pane and opens the Reason for Visit template, where you can indicate a follow-up visit or you can type a chief complaint. Document Reasons for a Patient's Visit

You can document up to six reasons for a patient's visit during intake. Open NextGen® Enterprise EHR. Open the Intake template, and then expand the Reason for Visit panel. To add reasons for the patient's visit without opening another template, select Do not launch HPI, and then do one of the following: Select the reason for visit from the left navigation bar. To add a customized reason, enter the reason in Other, and then select Add. Select Additional / Manage, select appropriate reasons from the Reason for Visit window, and then select Save and Close. Note: You can view all the available history of present illnesses (HPIs) by selecting the All hpis checkbox in the Reason for Visit window. To add reasons for the patient's visit with additional details by using another template, clear the Do not launch HPI checkbox, select the necessary reasons, and then select Save & Close . Set Up HPIs to Appear in the Reason for Visit

You can set up the History of Present Illness (HPI) to appear in the reason for visit. A practice can configure the Reason for Visit panel using the Framework Content system template. For more information on the reason for visit configuration, go to NextGen Healthcare Success Community, and download the latest Framework Content System Template for NextGen® Adaptive Content Engine. Open NextGen® Enterprise EHR. From the File menu, select System \ Practice Template. The Select Template window opens. In the Show section, select All. To open the Framework Content template, double-click Framework Content. Select Specialty and select a specialty from the list. Expand the Reason for Visit panel. In Filter, select the all option. Select the reason for visit linked to the appropriate History of Present Illness (HPI), which you want to add to the all list in the Reason for Visit list. Add Reasons for Visits

You can add HPIs as the reasons for a patient's visit. Open NextGen® Enterprise EHR. On the Intake template, in the Reason for Visit panel, select Do not launch HPI. Note: You can configure the Do not launch HPI checkbox on the Practice Configuration template. For more information, go to NextGen Healthcare Success Community, and download the Practice Configuration Template User Guide for NextGen® Adaptive Content Engine. To select the appropriate complaint, do one of the following: Select a link from the left pane in the Reason for Visit panel. Select Additional / Manage to choose maximum six reasons from the resulting template. Note: If the selected reason is a duplicate, a message appears highlighting the duplication.
