You can document participant information, program details, contact and location details, pharmacy, and relationship details on the CW Demographics (BH) template.
The CW Demographics (BH) template panel details are documented. In most of the panels, you can update or remove the details added in the grid.
Name of the panel | What you can do |

Participant Information | In this panel, the patient's details are displayed from the Patient's Demographic template, you can select Update to modify the information. You can select the state of birth, country of birth, race, and ethnicity of the participant. You can indicate if the foster child has pets that need to be brought to the foster home. The Family Unit Information grid shows information about the current members in the Family Unit. The Child Welfare program requires the family unit to include one family member as the main participant. |
Programs | You can use the Programs panel to enroll participants in a program and document information related to the participant’s programs. Throughout the NextGen® Behavioral Health Suite template set, the Program name list displays the programs that a participant has enrolled in.
In the Level of Care section, you can add the program, location, payer, provider, and starting and end dates in the tis_programs_hx - New Record window. In the Program Detail section, you can select the program detail name, start date, and end date of the program. and select Add to add the details to the grid. |
Contact Information | The patient's contact details are displayed from the Patient's Demographic template, you can select Update to modify the information. |
Employment | You can document the employment information for prospective foster parents. Select Add to add the employer name, contact number, occupation, employment status, hiring date, retired date, termination date, employment duration, reason for leaving, and the usual industry in the respective fields in the Employment/Occupation- New Record window. |
Places | You can track the important location details of a participant in the Places panel. The locations in this panel are in addition to the locations already recorded for the participant, such as employer location. You can select the place name, status, country, state, and country. You can enter the address, city, comments, zip code, phone number, email idID or website urlURL, and license number and then select Add to add the details to the grid. |
Address History | This panel provides the history of all addresses entered in the participant's chart. The primary address automatically fills data from the Patient's Demographic template. You can add the complete address history details of the patient using this panel. The address details are added to the grid. |
PCP/Primary Practice Provider/ Pharmacy | You can document the details of the participant's primary care physician and primary practice provider and their preferred pharmacy. In the PCP section, when you select the provider name, the name, address, and communication details of the selected provider fills in. Select the primary practice provider and enter the first visit and next visit dates of the participant. In the Pharmacy section, document the pharmacy details. |
Participant/Staff Relationship | In this panel, you can document details about individuals who are working with the participant in the foster care program and the role of each individual in the program. Select Add to open the Edit Patient Prov Role window. Select the provider details, program role, and date range.

External Relationship Information | You can document external relationships and emergency contact details for prospective foster parents. In this panel, indicate if the foster child is part of a sibling set.
Document the details in the External Relationships section.
In the External Relationships section, select Add to add the guardian details in the Guardian Info window. You can enter the relationship details of the guardian with the foster child. Enter the support role, name, dates of the support, primary, and secondary addresses, and also add the primary and secondary communication details of the guardian.
In the Emergency Contacts / Patient Relationship section, add the emergency contact of the participant's foster care program. |
Release of Info History | You can view all completed releases for a participant in the Release of Info History panel. Information in this grid is updated with the data from the Release of Info (BH) template. You can obtain, revoke, or disclose additional information on the Release of Info (BH) template. |
Family Household Information | You can collect information about all family members of the prospective foster parents and details about their vehicles before adding them to the family unit. This information helps you meet the requirements of the state child welfare department. In the Family Members section, to add a family member enter the start date and end date of the family member staying in the house, the full name of the family member and the member relationship with the main participant, date of birth, gender, race or ethnicity, and religion and select Add to add the details to the grid. In the Vehicle section, enter the start date, end date, make, model, year, and last 4 VIN, and select Add to add the details to the grid. |
Licensing/Certification Information | You can track the licensing and certification details for participants to ensure that they meet the requirements of the state child welfare agency. In this panel, select the Placement Type, enter the license or certification number and status of the participant, the dates on which the certification or license was submitted, issued, amended, or expired, enter the age, gender, and any restrictions, the capacity of the license. Indicate if the license is registered for adoption and the date of submission of the study. Select the appropriate option for the Respite, Medically Complex, ICPC, ORR, and Kinship fields and select Add to add the details to the grid. |
Placement Preferences/Kinship Information | You can document the placement preferences of the kids in the participant's family. You can provide the foster placement preference, adoption placement preference, kinship care, and add the details to the grid. |
Bed Placement Configuration | You can document details regarding availability for foster placement within the family.
In this panel, enter the room number and bed number for child placement. If a preference exists, enter the gender preference, race and ethnicity preference, preference for the age range of the foster child, and any other preference restriction. Select the appropriate option for sibling, child care, and pet considerations. You can select Add to add the details to the grid. |
Bed Placement | In this panel, you can assign the bed placement details. |
Tracking Information | You can document tracking details for the training that a participant undergoes during the foster care program.
You can select the family member, the member relationship fills in. You can enter the license or certification name, indicate if the item or license is required, and also enter the start date and end date of the license or certification, type, title, total hours, and renewal hours of the training, training date, and indicate if the training is completed. You can select Add to add the details to the grid. |
