Open the CW Demographics (BH) Template

You can document the demographic details of children and family members on the CW Demographics (BH) template. In NextGen® Enterprise EHR, select a patient, and open an encounter. On the Categories tab, select Open Template. The Select Template window opens. Select All. In the Demographics tab, select CW Demographics (BH). The CW Demographics (BH) template opens. Document Emergency Contacts and Patient Relationship Details

You can document emergency contact details that would play a role in the participant's foster care program. The data you enter into the Emergency Contacts/Patient Relationship section is also displayed in the Patient Information window. Open the CW Demographics (BH) panel. Expand the External Relationship Information panel. In the Emergency Contacts / Patient Relationship section, select Add. The Modify Patient Information window opens. Select Relations/Role. Modify Patient Information Window From the Relationship list, select New. The Relationship Lookup window opens. Do one of the following: To link the relationship to someone already in the system, search for the person, select it, and then select Open. To add a person to the system, enter the person's details, and then select New. The relationship record is added to the Emergency Contact / Patient Relationship grid. Document Pharmacy Details

You can document up to two pharmacies details of the participant. Open NextGen® Enterprise EHR. Open the CW Demographics (BH) template. Expand the PCP/Primary Practice Provider/ Pharmacy panel. Select Pharmacy. The Modify Patient Information window opens. Select Pharmacy from the left pane. Modify Patient Information Window Panel In the Pharmacy Info section, select the folder icon, and then select New. The Pharmacy Lookup window opens. To add pharmacy details, do one of the following: To link pharmacy details already in the system, search for the pharmacy, select it, and then select OK. To enter new pharmacy details, enter details of the pharmacy, and then select OK. Select OK. Note: You can add up to two pharmacy details. The pharmacy details are updated in the panel. Child Welfare Demographics

You can document relevant demographic information such as contact information, address and employment history, emergency contacts, and family household information. You can use the CW Demographics (BH) template to enroll users in programs and capture demographic and other important information on participants in a child welfare program. With the Family Unit feature, you can group patients under a certain name or category. Each member of the household, including foster children, must be entered into a family unit. Each family unit must have a single member identified as the main participant. To update family unit information, select File > Family Unit > Unit setup. Child Welfare Demographics If applicable, you can select the Special Assistance checkbox to open a picklist that can be configured on the BH Foundation Configuration template. Documenting Child Welfare Demographics

You can document participant information, program details, contact and location details, pharmacy, and relationship details on the CW Demographics (BH) template. The CW Demographics (BH) template panel details are documented. In most of the panels, you can update or remove the details added in the grid. Name of the panel What you can do Participant Information In this panel, the patient's details are displayed from the Patient's Demographic template, you can select Update to modify the information. You can select the state of birth, country of birth, race, and ethnicity of the participant. You can indicate if the foster child has pets that need to be brought to the foster home. The Family Unit Information grid shows information about the current members in the Family Unit. The Child Welfare program requires the family unit to include one family member as the main participant. Document Child Welfare Training and Group Information

You can generate a document with details about group sessions, trainings, participants, and course details that are scheduled for the child welfare program. Open NextGen® Enterprise EHR. Open the CW Training and Group Note (BH) template. Document the required information in each panel. Select Save and then Generate Document.
