Document Emergency Contacts and Patient Relationship Details

You can document emergency contact details that would play a role in the participant's foster care program. The data you enter into the Emergency Contacts/Patient Relationship section is also displayed in the Patient Information window. Open the CW Demographics (BH) panel. Expand the External Relationship Information panel. In the Emergency Contacts / Patient Relationship section, select Add. The Modify Patient Information window opens. Select Relations/Role. Modify Patient Information Window From the Relationship list, select New. The Relationship Lookup window opens. Do one of the following: To link the relationship to someone already in the system, search for the person, select it, and then select Open. To add a person to the system, enter the person's details, and then select New. The relationship record is added to the Emergency Contact / Patient Relationship grid. Document Child Welfare Demographics

You can document relevant demographic information such as participant information, address and contact information, placement preferences, and family household information. Open NextGen® Enterprise EHR. Open the CW Demographics (BH) template. Optional: Select the Special Assistance checkbox, and select the appropriate option. In each panel, document the required information. Select Save & Close. You can document the demographic details of children and family members on the CW Demographics (BH) template. In NextGen® Enterprise EHR, select a patient, and open an encounter. On the Categories tab, select Open Template. The Select Template window opens. Select All. In the Demographics tab, select CW Demographics (BH). The CW Demographics (BH) template opens. Documenting Child Welfare Demographics

You can document participant information, program details, contact and location details, pharmacy, and relationship details on the CW Demographics (BH) template. The CW Demographics (BH) template panel details are documented. In most of the panels, you can update or remove the details added in the grid. Name of the panel What you can do Participant Information In this panel, the patient's details are displayed from the Patient's Demographic template, you can select Update to modify the information. You can select the state of birth, country of birth, race, and ethnicity of the participant. You can indicate if the foster child has pets that need to be brought to the foster home. The Family Unit Information grid shows information about the current members in the Family Unit. The Child Welfare program requires the family unit to include one family member as the main participant. Document the PCP Communication Details

Open NextGen® Enterprise EHR, and then in Patient History, select Open Templates. The Select Template window opens. On the Medical Records tab, select All. Select PCP Communication (BH), and then select OK. Select the program, specialty, and visit type. Note: Only programs that the patient is actively enrolled in appear in the list. You can select <No Program> to create an encounter without a program, and then add the program later. In the Reason for Communication panel, document the reasons for the patient's communication with the PCP. In the Vital Signs panel, document the patient's vital signs such as height, weight, body temperature, blood pressure, respiration, and pain level. In the Allergies panel, review and update the patient's allergy details. In the Medications panel, review, update, and reconcile the patient's medications. Managing Pharmacy Information

On the Pharmacy tab, you can enter up to two preferred pharmacies for a patient. Pharmacy information includes name, address, and phone and fax numbers. The information is saved for established patients with an existing chart, and for new patients when the chart is created. Information on the Pharmacy tab is stored at the practice level; therefore, each practice can enter and maintain different information for a patient. Note: Pharmacies entered from the Patient Information window update the Patient Demographics template for the patient in NextGen® Enterprise EHR, and vice versa.
