Demographic Information for UDS

The following demographics information must be collected on the Demographics tab of the Add/Modify Patient Information window for all patients in NextGen® Enterprise PM: Birth Date/Age Sex Race Preferred Language Ethnicity Zip Code The following UDS information must be collected on the UDS tab of the Add/Modify Patient Information window for all patients in NextGen® Enterprise PM: Homeless Status Migrant Worker Status Language Barrier Public Housing Primary Care School Based Health Center Veteran Status Primary Medical Coverage Note: The Race and Ethnicity fields can display on either the Demographics tab or the UDS tab of the Add/Modify Patient Information window. In Enterprise Preferences > Client Defined, select Display General to display race/ethnicity on the Demographics tab, or select Display UDS to display race/ethnicity on the UDS tab. Information Needed for the UDS Report

Although some data required for the UDS Report is more readily (or only) available from a general ledger or other external system, much of the data can be reported directly from NextGen® Enterprise PM. Several demographic fields required by Health Centers for the UDS report are available on the UDS tab of the Add/Modify Patient Information window. Additional fields are available for use by IHS practices. To display the UDS tab, enable the Display person UDS fields check box in Enterprise Preferences > UDS. UDS Information

In the UDS section of the UDS tab, you can enter information that is not part of the standard demographics record for a person or patient. This is typically used by Community Health Centers to capture information such as homeless status and migrant worker status. It is also used by Indian Health Systems to capture information such as tribal affiliation and blood quantum. UDS information is stored at the system level; therefore, all practices in all enterprises share the same information for a person or patient. UDS Tab

The UDS fields are only used for UDS (Uniform Data System) reporting. The fields display in NextGen® Enterprise PM on the Patient Information window only when they are enabled in Enterprise Preferences. You can also require that users enter information in these fields. Enabling and Requiring UDS Fields

Enabling UDS Fields When UDS fields are enabled, the UDS tab displays on the Add/Modify Patient Information window in NextGen® Enterprise EHR and NextGen® Enterprise PM. Users can then access the tab to select the appropriate UDS values for each person/patient. UDS fields are enabled at the enterprise level. Therefore, if enabled, the UDS tab displays on the Add/Modify Patient Information window in all practices within the enterprise. Requiring UDS Fields UDS fields can be set up to be required at either the enterprise level or at the practice level. If required at the enterprise level, users in all practices in the enterprise must enter data in the UDS fields for all persons (globes) and all patients (charts) when the demographic record is created for a person/patient.
