Demographic Information for UDS

The following demographics information must be collected on the Demographics tab of the Add/Modify Patient Information window for all patients in NextGen® Enterprise PM: Birth Date/Age Sex Race Preferred Language Ethnicity Zip Code The following UDS information must be collected on the UDS tab of the Add/Modify Patient Information window for all patients in NextGen® Enterprise PM: Homeless Status Migrant Worker Status Language Barrier Public Housing Primary Care School Based Health Center Veteran Status Primary Medical Coverage Note: The Race and Ethnicity fields can display on either the Demographics tab or the UDS tab of the Add/Modify Patient Information window. In Enterprise Preferences > Client Defined, select Display General to display race/ethnicity on the Demographics tab, or select Display UDS to display race/ethnicity on the UDS tab. Additional fields may be used by IHS practices to meet state specific and/or other reporting needs. These fields are not required for the UDS report. Information Needed for the UDS Report

Although some data required for the UDS Report is more readily (or only) available from a general ledger or other external system, much of the data can be reported directly from NextGen® Enterprise PM. Several demographic fields required by Health Centers for the UDS report are available on the UDS tab of the Add/Modify Patient Information window. Additional fields are available for use by IHS practices. To display the UDS tab, enable the Display person UDS fields check box in Enterprise Preferences > UDS. IHS Specific Master Lists

When enabled, a UDS tab displays on the Add/Modify Patient Information window in NextGen® Enterprise PM. The tab includes fields for capturing additional patient demographic information used by IHS practices for reporting purposes. The UDS tab and related IHS fields are enabled at the enterprise level. Therefore, they are available to all practices in the enterprise. The pick-list items available for user selection in each IHS field are created in the corresponding Master Lists tables in File Maintenance.
