The following demographics information must be collected on the Demographics tab of the Add/Modify Patient Information window for all patients in NextGen® Enterprise PM:
- Birth Date/Age
- Sex
- Race
- Preferred Language
- Ethnicity
- Zip Code
The following UDS information must be collected on the UDS tab of the Add/Modify Patient Information window for all patients in NextGen® Enterprise PM:
- Homeless Status
- Migrant Worker Status
- Language Barrier
- Public Housing Primary Care
- School Based Health Center
- Veteran Status
- Primary Medical Coverage
Note: The Race and Ethnicity fields can display on either the Demographics tab or the UDS tab of the Add/Modify Patient Information window. In , select Display General to display race/ethnicity on the Demographics tab, or select Display UDS to display race/ethnicity on the UDS tab.
