Re-Evaluation Charge - PT/OT Visit

On a PT or OT visit type, if the Provider did not mark the Re-Evaluation Charge checkbox at the top of the Objective panel grid on Single Visit, but you need to bill a Re-Eval CPT code, select the Re-Eval Charge checkbox above the Charges grid on Fee Sheet. This also edits the Visit Type in the document header to display as Re-Evaluation. Parent topic: Fee Sheet

Select Visit Type Complexity

For visits that require a complexity code (PT/OT) or an E&M code (acupuncture), options appear below the Visit Type. Select the appropriate option. Re-Evaluation Charge

If you select the Re-Eval Charge checkbox on the Fee Sheet template for a chiropractic visit type, it changes the visit type on the Office Note document to a Re-evaluation. This checkbox does not alter any billable charges. Parent topic: Fee Sheet - Chiropractic Visit

Re-Evaluation Checkbox

The Re-Evaluation Charge checkbox above the grid on the Single Visit template is used when the visit type is Daily or Progress. If you determine that you have documented enough data to support billing for a re-evaluation (97164 or 97168), you can select the Re-Evaluation checkbox to send the Re-Eval code to the Fee Sheet template for the visit charge. Re-Evaluation Charge checkbox If you select the Re-Evaluation Charge checkbox, the following happen: The Visit Type on the office note displays as “Re-evaluation” regardless of the selected visit type. If there has been a significant change in the patient’s status that requires additional documentation, you can bill a re-evaluation charge. To indicate that an encounter qualifies for re-evaluation, mark the Re-Evaluation Charge checkbox on the Objective panel above the Exam grid. Parent topic: Objective Panel

Fee Sheet

After you have completed all documentation for an encounter, select the Fee Sheet tab in the workflow bar, or the Fee Sheet button below the panels on the Single Visit template, to open the Fee Sheet template. Fee Sheet on the navigation bar When you open the Fee Sheet, the system uses the documentation on that encounter to determine which CPT codes and the total number of units can be billed. It takes into consideration any payer-specific billing rules configured in the Billing Configuration system template and adjusts billing as needed, based on the payer attached to the case.
