Re-Evaluation Checkbox

The Re-Evaluation Charge checkbox above the grid on the Single Visit template is used when the visit type is Daily or Progress. If you determine that you have documented enough data to support billing for a re-evaluation (97164 or 97168), you can select the Re-Evaluation checkbox to send the Re-Eval code to the Fee Sheet template for the visit charge. Re-Evaluation Charge checkbox If you select the Re-Evaluation Charge checkbox, the following happen: The Visit Type on the office note displays as “Re-evaluation” regardless of the selected visit type. Re-Evaluation Charge - PT/OT Visit

On a PT or OT visit type, if the Provider did not mark the Re-Evaluation Charge checkbox at the top of the Objective panel grid on Single Visit, but you need to bill a Re-Eval CPT code, select the Re-Eval Charge checkbox above the Charges grid on Fee Sheet. This also edits the Visit Type in the document header to display as Re-Evaluation. Parent topic: Fee Sheet

Second Evaluation N/A Checkbox

If a second evaluation is completed within the same case, a N/A checkbox appears over the Treating Dx grid. The system requires that you enter a new treating diagnosis on an Eval visit type. However, if a new diagnosis does not apply to the patient scenario, you can select the N/A checkbox to be able to submit charges.
