Chief Complaints

You can document up to six chief complaints for a case. These complaints appear in the blue fields below Patient Complains of. Patient chief complaint fields on the Speech Subjective panel You can take two approaches to adding chief complaints to the History of Complaint(s) grid: You can enter all complaints into the list, and then select each one, add its details, and then add it to the History of Complaint(s) grid. You can add one complaint at a time to the list, select its details and move it to the grid, then add another to the list and repeat the process. Add Complaints to the List Add patient chief complaints to the Speech Single Visit Subjective panel. View a Complaint from the Grid You can view a complaint on the grid of the Speech Single Visit Subjective panel. Past History

You can enter Past History information by selecting any of the links in the left navigation for Function, Nutrition, Pediatric, Lymph/Oncology, Cognition, and Fluency. Past History grid When you have opened any Past History template, you can use the left navigation to jump to another template. Note: To fill in these templates more efficiently, you can use Get Data to pull information from prior visits, or QuickPicks to enter saved favorites. See Use Get Data and QuickPicks for more information. After you have entered all information on a template, select Save & Close. All information is stored in the Past History grid. Audiology Subjective Panel

The Ordering Diagnosis grid pulls data entered on Homepage. To update Ordering Diagnosis information, you must do so on Homepage. The Past Medical History grid displays any medical and surgical history that was captured in a NextGen Healthcare application for the patient. You can edit the Past Medical History on this page and add family and social history. Documenting a Patient's History Summary

You can add, update, delete, and review a patient's history summary details from the History Summary panel. You can document a patient's history details in the History Summary panel in the following categories: Past Medical/Surgical Interim Social Family Diagnostic The History Summary panel also displays the patient history documented by the intake staff on the Histories template. You can add diagnostic studies with a documented Result or Interpretation to the grid in the Reason for Visit panel by selecting the Add to HPI Grid button. The grid includes a list of categories on the left pane to select and view the documented data. You can filter the patient history records by selecting the Medical History or Surgical History option for Past Medical/Surgical. You can view and enter confidential details provided by the patient during the course of the encounter using The Confidential link. Past Medical and Surgical History

The Past Medical History grid displays any Medical and Surgical history that was captured in a NextGen Healthcare application in your organization or practice. For example, if you have one practice but multiple locations, all locations can see the Past Medical History regardless of where it was entered. Single Visit Subjective Panel

Use the Subjective panel on the NTS Single Visit template to document the patient’s history. The Subjective panel also gives you access to additional templates to document the patient’s limitations, work demands, and specialty subjective information. In the Job Title, Work Status, and Last Worked fields, you can document the patient’s workplace and work status information. Note: These fields share the same values from the Job Title, Work Status, and Last Worked fields on the NTS Homepage template. The Ordering Diagnosis grid pulls data entered on the NTS Homepage template. To update Ordering Diagnosis information, open the NTS Homepage template.
