Add Past Medical History
The Past Medical History grid displays any history that has been captured within that practice. For example, if you have one practice and multiple locations, all locations see the Past Medical History regardless of where it was entered. Select the Edit button below the grid. The NTS Sp PMH template for past medical history opens. NTS Sp PMH template for past medical history Items listed in black prompt for an onset date. Items listed in blue open a picklist to further specify the PMH for that item and then prompt for an onset date. Select the checkbox next to the item you want to enter. If the item is blue, select the more specific description from the picklist, and then select OK. Enter the Onset Date, if known, and then select the side, if applicable. Add Past Medical History from a Search

Enter a search term in the top field, and then select Search. Search results for other past medical history Highlight the item from the list, and then select Select. The window closes and the item you selected enters the Description field in the NTS Sp Pmh Select window, along with the SNOMED code. NTS Sp Pmh Select template Enter the Onset Date. Select the Side field to select the side if applicable. Select Save to Grid. The selected item appears in the grid on the NTS Sp PMH template. Select Save & Close. Add Past Medical History
Select the Edit button below the Past Medical History grid. The NTS Sp PMH template opens. NTS Sp PMH template Mark the checkbox next to an item you want to enter. If you select an item with black text, a date field appears for you to enter the onset date. If you select an item with blue text, the Ainf Sp Pmh Select Dbpl picklist opens for you to further specify the item and enter an onset date. Select the more specific description from the picklist, and then select OK. The NTS Sp Pmh Select window opens. NTS Sp Pmh Select template Enter the Onset Date. Select the Side field, and then select the side if applicable. Select Save to Grid. The item enters the grid on the NTS Sp PMH template. Repeat step 2 for any additional medical history you want to add. Select Save & Close. All history items entered on the Past Medical History template appear in the Past Medical History grid on the Single Visit template. Parent topic: Past Medical History

Add Medical History
Select the Medical HX button below the grid. The Past Medical History template opens. Select the checkbox next to the history you want to add. Items in black prompt for an onset date. Items with blue labels open a picklist to select from, to further specify the PMH for that item. For example, if you select Cancer, a picklist opens to select the type of cancer. Select the type of cancer, and then select OK, where upon you are prompted to enter the onset date. If you select Other, a search window opens. Here you can search for the specific PMH. Select the desired PMH, enter the onset date and then select Save to Grid. Select Save & Close. Parent topic: Past Medical and Surgical History

Past Medical History
Any past medical history entered in the NextGen® Enterprise EHR appears in the Past Medical History grid. You can add additional items to it from the NTS Sp PMH template. You can also add family and social history for the patient, but those do not appear in the grid. Add Past Medical/Surgical History
On the Bh Hx section tab, click the Add button under the Past medical/surgical/interim history grid to access the Past Medical History template.By default, the application fills the Specialty field with the current specialty. Note: The Configure Medical Surgical and Family History system template is used to configure the conditions, surgical procedures, sub-items of a condition/procedure, management items, and associated SNOMED® codes. To add an item to the Medical or Surgical panel, select the respective checkbox for the condition or surgery.The Onset Date (in the Medical panel) or Date (in the Surgical panel) field and the Details icon appear beside the selected item. Use the Onset Date field in the Medical panel to enter the date when the condition first appeared. Use the Date field in the Surgical panel to enter the date the surgery was performed. To add problems that are not available on the Past Medical History template, use the Other field. Click the Onset Date or the Date field
