Add SOAP Comments to the Rapid Flowsheet

In the Rapid Flowsheet template, you can enter SOAP (Subjective, Objective, Assessment, and Plan) comments for a patient's treatment by typing them and by using saved template entries (My Phrases text). You can also review comments from previous encounters and copy them to today’s visit. To save time, you can enter these comments in advance of an appointment with a Booked status.

Open the NTS Rapid Flowsheet Worklog template, and then select an appointment row in the grid.
The Rapid Flowsheet template opens. The patient’s name, sex, and birthdate appear in the panel headers.
Expand the Additional Comments panel to view the SOAP comments fields.
Manually enter the comments.
To insert saved template entries at the end of any text already in the field, select My Phrases, select a phrase from the list, and then select OK. To append more phrases, select My Phrases again.
Note:
- Although you cannot add or edit saved phrases in the list, you can edit them after you insert them in the comments field.
- If the inserted phrase causes the comments field to exceed the maximum character limit, the phrase is truncated.
If a date appears above the comments field, then comments exist from the last encounter, and you can do the following:
- To preview the previous comments in a tooltip before copying them, hover over the information icon next to the Last Visit date.
- To copy comments from the patient's last encounter, select Copy. If the field already contains text, the copied comments are appended to the existing text. The system copies data from the most recent encounter within the case that has an appointment.
Do one of the following:
- If the appointment status is Booked and you have finished entering data for now, select Save & Close. You can reopen the template to make changes later on.
- If you want to document exercises, expand the Treatment panel.
- If you have completed the documentation, finalize and send the flowsheet data to the chart.
