Add Details to a Complaint
You can capture specifics about an individual complaint. All fields that document details about the complaint are optional.
Under
Patient Complains of
, select the option for the complaint you want to modify.
You can document specifics only for the selected complaint.
Under
Onset Factors
and
Previous Treatments
, select the relevant checkboxes to add details about the complaint.
You can enter details of
Alleviating Factors
,
Aggravating Factors
,
Behavioral Characteristics
, and
Current Status
into those fields.
In
Additional Details
, enter any other information related to the complaint.
Select
Add
to save the information to the
History of Complaint(s)
grid.

Add Complaints to the List
Below Patient Complains of, select the option for the blank field you want to modify. Select the white field. A picklist of complaints opens. Chief Complaint picklist Select a complaint from the picklist. Note: If you select the blank space at the top of the picklist, you can enter an entry into the selected field. The complaint displays in the blue field, and the option for the next field is selected. If necessary, repeat the previous steps to add more complaints. Edit a Complaint
In the History of Complaint(s) grid, select the complaint. Select Edit located below the grid. The NTS PT Injury Edit Pop template opens. AINF PT Injury Edit Pop template If you entered the details for the wrong complaint, you can select the Chief Complaint field, and then select a different complaint. Chief Complaints
You can document up to six chief complaints for a case. These complaints appear in the blue fields below Patient Complains of. Patient chief complaint fields on the Speech Subjective panel You can take two approaches to adding chief complaints to the History of Complaint(s) grid: You can enter all complaints into the list, and then select each one, add its details, and then add it to the History of Complaint(s) grid. You can add one complaint at a time to the list, select its details and move it to the grid, then add another to the list and repeat the process. Add Details to a Complaint
When capturing specifics about an individual complaint, be sure that the option next to that complaint is selected. Select the checkboxes to the right of the Patient Complains of fields to add details about the complaint. Document specifics only for that complaint. Fields for patient complaints As needed, select the No or Yes option for any Recent Changes in the patient’s medical status or Medications. Add Complaints to the List
Select in the white field below Patient Complains of. The picklist of complaints open. Select an item from the picklist or select the blank at the top of the picklist to type into the field. Chief Complaint picklist When you have entered a complaint, it moves to one of the blue fields, and the option for the next blank field is selected.
