This grid displays who is authorized to receive PHI for the patient.
In the
Authorized Release of PHI
grid, double-click in the grid.
This opens the
IORTSAuthReleasePHI
template.
Enter the
Start Date
and
End Date
for authorization.
Select who authorized the release of PHI in the
Authorized By
field.
Enter to whom authorization is granted, in the
Authorization To
field.
Click the
Relation
field to select the relationship to the patient of the person who is being granted authorization.
Enter the phone number of the person to whom authorization is granted.
Select whether authorization is
Active
or
Inactive
, and if inactive, enter the date it was made inactive.
Click
Save
.
Click
Close
.
The information entered is shown in the
Authorized Release of PHI
grid.

Document PHI Restrictions
The Restrictions displays shows what PHI is restricted and from whom it is restricted. In the Restrictions grid, double-click to open the IORTSRestrictions template. IORTS Restrictions window Enter the Date. Select the an option to indicate whether the PHI is restricted from an Individual or in General. Click the Restriction field to select what information is restricted from a multi-pick picklist. Click Save. Click Close. The information you entered is shown in the Restrictions grid. Medical Records Request and Disclosure Log
The Medical Records Request and Disclosure Log grid displays records that have been requested and to whom they have been disclosed. Double click the Medical Records Request and Disclosure Log grid to open the IORTSreq_disclosure template. IORTS Request Disclosure window Enter in the date the request was received in the Date Received field. Click the Med Records Release Form field and select whether the patient signed or refused to sign. Click the Records Requested field and select all records requested from the multi-select picklist. Select the format of the records disclosed (Paper, Electronic, etc.). Enter who disclosed the records. Enter the Date of Disclosure. Enter the contact information for the person or entity to whom the records were disclosed. Select Completed. The user name enters the field, with a date and timestamp. Click Save. Document Details for the Release of Information to the Patient
Open NextGen® Enterprise EHR, and then in Patient History, select Open Templates. The Select Template window opens. On the Medical Records tab, select All. Select Psychiatric Advance Directive (BH), and then select OK. Select the program, specialty, and visit type. Note: Only programs that the patient is actively enrolled in appear in the list. You can select <No Program> to create an encounter without a program, and then add the program later. In the Release Info panel, document the authorization details, reason for the disclosure, and the contacts involved in the release. In the Documents panel, document the details of the nature of information to be released. Document whether the patient received any privacy notice. Select Generate Document. Add Chart Details
You can document chart details as a part of the patient's contact details. Open the Patient Contact template. Chart Information section on the Patient Contact template In the Chart Information section, select the chart type. Click the Taken By field. The name of the user and date and time of the entry appear in the corresponding fields, and the get providers window opens. Select the provider, and click OK. The Provider field in the Provider Information section is automatically filled. If the patient has been contacted on an emergency basis, select the STAT option. Click the Type field, and select the contact type from the list. The Who field displays a list to select the relationship of the person who provides the patient details. Enter a New Authorization by Units
Select the payer from the Insurance Information grid. Select the Units option below the Precertification/Authorization grid. Use the fields below the Precertification/Authorization grid to enter the authorization information. You can use the Get button to populate the fields with authorization information that has already been attached to the Insurance in NextGen® Enterprise PM. Select Get and then select the appropriate authorization from the pop up to pull into the Precert template fields. Note: Pre-Cert Company, Associated Diagnosis, and Expiration Date are optional. The Associated Diagnosis grid will only display Dx codes after the therapist has established the treating diagnosis. Using the grid that displays CPT codes, select the CPT codes that pertain to the authorization. All selected CPT codes display a checkmark next to them at the top of the grid.
