displays shows what PHI is restricted and from whom it is restricted.
In the
Restrictions
grid, double-click to open the
IORTSRestrictions
template.
Enter the
Date
.
Select the an option to indicate whether the PHI is restricted from an
Individual
or in
General
.
Click the
Restriction
field to select what information is restricted from a multi-pick picklist.
Click
Save
.
Click
Close
.
The information you entered is shown in the
Restrictions
grid.

Record the Authorized Release of PHI
This grid displays who is authorized to receive PHI for the patient. In the Authorized Release of PHI grid, double-click in the grid. Authorized Release of PHI window This opens the IORTSAuthReleasePHI template. Enter the Start Date and End Date for authorization. Select who authorized the release of PHI in the Authorized By field. Enter to whom authorization is granted, in the Authorization To field. Click the Relation field to select the relationship to the patient of the person who is being granted authorization. Enter the phone number of the person to whom authorization is granted. Select whether authorization is Active or Inactive, and if inactive, enter the date it was made inactive. Click Save. Medical Records Request and Disclosure Log
The Medical Records Request and Disclosure Log grid displays records that have been requested and to whom they have been disclosed. Double click the Medical Records Request and Disclosure Log grid to open the IORTSreq_disclosure template. IORTS Request Disclosure window Enter in the date the request was received in the Date Received field. Click the Med Records Release Form field and select whether the patient signed or refused to sign. Click the Records Requested field and select all records requested from the multi-select picklist. Select the format of the records disclosed (Paper, Electronic, etc.). Enter who disclosed the records. Enter the Date of Disclosure. Enter the contact information for the person or entity to whom the records were disclosed. Select Completed. The user name enters the field, with a date and timestamp. Click Save. Document the Reason for Visit
The Reason for Visit panel is where the reason for visit, as described by the patient, is documented, along with an initial History of Present Illness (HPI). The provider should review reasons for visit, update if necessary, and complete HPI information. Reason for Visit If the Introduction field contains text, review the content and make any necessary changes. The introduction appears as the first sentence of the visit note, and it can be edited for the individual patient. A typical introduction might contain the patient's age, gender, and the reason for visit. If the introduction field is blank, compose an introduction for the patient. If checked, click to clear the Do not Launch HPI check box. In the Reason for Visit data grid, click one of the reasons for visit entered previously to open the HPI template to add additional details. Document HPI of Patients
You can document a history of present illness (HPI) for chief complaints. On the Billable Visit template, in the History panel, click on any chief complaint. The IORTShpi window opens displaying the corresponding HPI tab. IORTS HPI template Note: To document a different chief complaint, you can click the desired HPI tab. The Procedures grid displays the details from the Orthopedic Surgical History section on the Billable Visit template. Do one of the following to enter the patient's HPI. Click Get Data, select the desired encounter to copy the details from the scope window, and then click OK. In the QuickPick toolbar, click the Star icon or Double star icon icon, select the desired set name from the filter window, and then click OK. Select the appropriate options and enter the details in the fields. To add details of the patient's employer, click Add below the Employer field. Create Restrictions Notes
You can create a patient's restrictions notes for gym and sports activities. Open the Forms template. Restrictions panel on the Forms template Select the Pt Seen check box. A window opens for selecting the date of the appointment. Select the date on the calendar window, and click OK. To enter the time of the appointment, do one of the following: To enter the current time, click the Clock icon icon. To enter a specific time, click the Down arrow icon icon, enter the time in the time entry window, and then click OK. In the Restrictions panel, do the following: In the No participation in section, select the activity that patient cannot participate in. A window opens for selecting the duration. Enter the duration of the patient's inactivity, and click OK. Note: You can also indicate if the patient cannot participate in the selected activities until cleared by provider or next appointment.
