This section captures your plans to treat the patient for today’s visit. It includes any verbal counseling done, medications, office procedures, diagnostic tests, labs, DME’s, patient education, etc. and an area to designate follow-up visits. These fields are all picklist-driven and fully configurable within the Picklist Configuration template. On the Finalize Visit template, select the Incident To check box. The IORTS Incident to Plan window opens. You can filter the grid by selection the following option: Time Period Current Provider All Providers All Encounters Plans Only-ones that had plans Add additional diagnoses in the Today's Diagnosis grid if your practice is configured to allow it. Note: If new diagnoses are added in this manner there must be documentation on the Billable Visit template per CMS guidelines. Today's Diagnosis grid on the Finalize Visit template Select the encounter which today’s visit is incident to. Click OK. If the Add New Task window opens, do one of the following: To close the window without sending a task, click Cancel. To send a task, select any desired recipients (for example, billing department), move them to the Task Recipients grid, and then click Task. A field appears next to the Incident To check box, showing the encounter date that this visit has been designated as incident to. Entering a plan includes documenting Management (for example, medications, office procedures, DME), Diagnostics (procedures, tests, labs, consults) Instructions to the patient, and Printed Materials given to the patient. This panel also has an area to enter Verbal Counseling and one to designate Next Visit time frame and X-rays. The fields (except Verbal Counseling) are picklist-driven and configurable in the Picklist Configuration template. You can enter data in these ways: Click fields to select items from configurable picklists. Click on the star or double star icon in the QuickPick Toolbar at the top of the Non-Billable Visit template to load a template default. Type text in the Verbal Counseling fields to describe verbal counseling. Once all information has been entered, sign off on the template (if not utilizing PAQ functionality) and click the Finalize button to open the Finalize Visit template to submit charges. The Today’s Diagnosis grid pulls in diagnoses that were entered on the Billable Visit template under the Diagnosis Management section. Diagnoses with a check mark are selected to be applied to Procedure codes in the coding grid. All diagnoses are selected by default when you open the Finalize Visit template. Click the Add - Remove button below the Today’s Diagnosis grid. The IORTS FV Add Assess template opens. IORTS FV Add Assess template Double-click the diagnosis in the patient’s Diagnosis grid to add it to the Today’s Diagnosis grid. Note: If a diagnosis is not in the grid, you can find and add it by using the Add button (or the Problem IT button if your practice is configured to use IMO). You can enter data multiple different ways: Click each field to select items from configurable picklist. Click on the star or double star icons in the QuickPick Toolbar at the top of the Fracture Care template or from the MDM button that opens the MDM template to load a template default. Click the MDM button to launch a template where you can use the Get Data button to load MDM information from previous encounter. The Diagnosis grid displays the patient’s diagnoses. You can add a diagnosis for today’s visit or use one from the Diagnosis grid. Filter the grid by selecting the Orthopedic or Medical option above the Diagnosis grid. Click Add under the grid to launch the Common Assessment template or click Problem IT if using IMO. Select a diagnosis to add to the patient’s history. Click Save & Close. Select a diagnosis from the Diagnosis grid to add it to today’s plan. The selected diagnosis moved to the Diagnosis field. Note: For the multi-fracture template, you can select the second and third diagnosis from the Diagnosis grid and the diagnosis are assigned to the Fracture 2 and Fracture 3 field respectively.
