Enter Plan Information

Entering a plan includes documenting Management (for example, medications, office procedures, DME), Diagnostics (procedures, tests, labs, consults) Instructions to the patient, and Printed Materials given to the patient. This panel also has an area to enter Verbal Counseling and one to designate Next Visit time frame and X-rays. The fields (except Verbal Counseling) are picklist-driven and configurable in the Picklist Configuration template. You can enter data in these ways: Click fields to select items from configurable picklists. Click on the star or double star icon in the QuickPick Toolbar at the top of the Non-Billable Visit template to load a template default. Type text in the Verbal Counseling fields to describe verbal counseling. Once all information has been entered, sign off on the template (if not utilizing PAQ functionality) and click the Finalize button to open the Finalize Visit template to submit charges. Assessment and Plan (MDM) Panel

The Assessment and Plan panel enables the provider to select the diagnosis for today’s plan, and enter today’s plan, next visit information, and verbal counseling. Assessment and Plan panel on the Non Billable Visit template The Diagnosis grid shows the patient’s diagnoses. The system defaults to the Orthopedic option to view Orthopedic-specific diagnoses. Enter Assessment Plan Details

Open NextGen® Enterprise EHR. On the Assessment Plan Details template, select the A/P Details tab. To add or change a diagnosis in the Today’s Assessments grid, select Add or Edit to open the Add or Update Assessment template. To change the current working assessment, in the Today's Assessments grid, select the diagnosis being addressed for this encounter. The selected diagnosis appears in the Selected Dx field. It becomes the new working assessment that carries forward to the other templates that share the assessment. Enter details manually or use My Phrases or Common Phrases for the impression, differential diagnosis, and patient and provider plans in the respective sections. To enter follow-up appointment and disposition information, select Follow Up. To document counseling educational details and factors, select the Counseling Details link. Filter and View Plans for Chronic Conditions in the Medical Summary

Open NextGen® Enterprise EHR. In an encounter, select any Specialty and Office Visit as Visit Type. Select My Plan link, and then select the Assessments tab. Use the Add common assessment/Diagnosis Code Lookup link to add a few assessments. Select Save & Close to return to the Add or Update Assessment template. Select Save & Close to return to the previous template. On the Intake tab, in the Reason for Visit panel, select Chronic Conditions. Select a diagnosis from Today’s Assessment grid. On the Medical Chart Summary section, select the Plans link. The application automatically fills the plans for all chronic conditions in the grid. Chronic Conditions To view only specific assessments, select the Filter by Diagnosis field to select a diagnosis from the resulting list. The grid displays only the assessments related to the selected diagnosis. To fill the grid with assessments rendered by a specific provider, select the Filter by Provider field to select the appropriate name from the res

Add a Diagnosis to Plan

The Diagnosis grid displays the patient’s diagnoses. You can add a diagnosis for today’s visit or use one from the Diagnosis grid. Filter the grid by selecting the Orthopedic or Medical option above the Diagnosis grid. Click Add under the grid to launch the Common Assessment template or click Problem IT if using IMO. Select a diagnosis to add to the patient’s history. Click Save & Close. Select a diagnosis from the Diagnosis grid to add it to today’s plan. The selected diagnosis moved to the Diagnosis field. Note: For the multi-fracture template, you can select the second and third diagnosis from the Diagnosis grid and the diagnosis are assigned to the Fracture 2 and Fracture 3 field respectively. Add Assessments or Plans

Open NextGen® Enterprise EHR. Open the SOAP template. Click the Assessment/Plan panel. From the left pane, click an assessment category link. The diagnosis search window displays a list of diagnoses for the category selected. Select an appropriate diagnosis and then click Select. The diagnosis has been added to and displayed in the grid with the letter “D” and lines for A and P. Assessment/Plan Panel Click the A row in the grid for a diagnosis. The Assessment And Plan template opens. Assessment And Plan Template Note: Certain diagnosis requires you to choose a category before documenting the details.
