Apply Diagnoses to Procedures
To add all selected diagnoses to all procedures in the bottom grid, click the
Apply To All
button.
Any diagnosis with a check mark is added to all procedure in the coding grid.
To apply codes to individual procedures, do the following:
Select the diagnosis codes desired for the procedure.
Select the procedure.
Click the

button.

Coding Procedures
You can view all procedures, diagnoses, and modifiers that need to be billed for the current encounter to finalize a visit. The coding grid on the Finalize Visit template displays the following items that need to be billed for the current encounter. Procedures Diagnoses Modifiers Orders for durable medical equipment (DME) that are dispensed by your practice The following details are displayed for each item in the coding grid. Visit type Billing status of the procedure Description of the procedure to be billed Primary diagnosis codes after it is been coded Modifiers Code being sent for billing Coding grid on the Finalize Visit template The billing status is displayed as Coded for the items that have a diagnosis code, and the billing status is blank for the items without a diagnosis code. After you submit the charges, the billing status is displayed as Submitted for all items in the coding grid. Add an Assessment (Diagnosis)
The Today’s Diagnosis grid pulls in diagnoses that were entered on the Billable Visit template under the Diagnosis Management section. Diagnoses with a check mark are selected to be applied to Procedure codes in the coding grid. All diagnoses are selected by default when you open the Finalize Visit template. Click the Add - Remove button below the Today’s Diagnosis grid. The IORTS FV Add Assess template opens. IORTS FV Add Assess template Double-click the diagnosis in the patient’s Diagnosis grid to add it to the Today’s Diagnosis grid. Note: If a diagnosis is not in the grid, you can find and add it by using the Add button (or the Problem IT button if your practice is configured to use IMO). Add a Diagnosis Code for Diagnosis
Select OK on the alert message. Select the procedure billing details in the grid. The data appears in the corresponding fields above the grid. Do one of the following: Select the Dx Description 1 and select the diagnosis code from the list. Select Cancel on the list. The diagnosis search window opens. Search and select the required diagnosis.The selected diagnosis and the associated code appear in the Dx Description 1 and Code fields respectively. If applicable, repeat the above steps for Dx Description 2 through Dx Description 4 to add additional diagnosis. Note: The diagnosis selected in the Dx Description fields uploads in the Today's Assessments grid on the Add or Update Assessment template. Select Update.If you add additional data in the grid, the Verify Charges button appears in the Procedure Billing panel. Note: If the value is not associated with a procedure code, then the system uploads the default procedure billing code available for each procedure in the Procedure Documentat

View and Change the Order of Diagnoses Codes
You can view diagnosis codes attached to a procedure and change the order in which those codes are displayed for billing purposes. On the Finalize Visit template, click the View button under the grid displaying the Visit Type, Billing Status, Description, and more. The IORTS FV View template opens. IORTS FV View template Select a procedure from the Procedures grid to view the diagnosis codes. A diamond next to the procedure indicates the selected procedure. The fields below the grid fill with the corresponding data from the grid. The lower left section displays ICD codes attached to the selected procedure. To change the order of the codes, select a diagnosis code and use the up and down arrows to move the code up or down in the list. The code at the top is the primary ICD code. Attach a Suggested Modifiers
You can attach suggested modifiers to a procedure in the coding grid. Suggested modifiers appear above the grid in blue text. Different categories allow for different modifiers which appear above the grid when a procedure is selected. For example, the office visit procedure code allows for the following common modifiers: Modifiers on the Finalize Visit template Select the procedure from the coding grid. Add a Diagnosis
Click the Add-Remove button below the diagnoses grid to launch the Add/Remove Assessments template. Double-click the diagnosis in the Diagnosis grid. Note: If the diagnosis is not in the Diagnosis grid, you can add it using the Add button to launch the Common Assessment template or Problem IT button to launch IMO template. Click Close. Apply the diagnoses to the procedures in the coding grid by doing one of the following: Click the Apply to All button to add all selected diagnoses to all procedures in the bottom grid. Any diagnosis with a check mark will be added to all procedures in the coding grid. To apply a code to a specific procedure, by selecting them (anything with a green check mark), selecting the procedures in the Procedures grid, and clicking Apply Codes.
