Create Goals for the Low Vision Patient
Optometrists can create goals for the low vision patients in the Goals panel of the LV Devices - OPT template. In the Goal panel, enter the Name of the goal. Add Notes related to the goal. Enter the date the goal was Added for the patient. Enter the Target date for the goal. Click Add to add the goal to the grid. Modify Low Vision Goals You can modify goals for the low vision patient on the LV Devices - OPT template. Low Vision Devices
You can document and create a comprehensive, individual rehab plan for the low vision patient based on the patient's visual abilities, goals, and devices on the LV Devices - OPT template. Goals and devices can be monitored and updated in future encounters. Create Goals for the Low Vision Patient You can create custom goals for the low vision patient on the LV Devices - OPT template. Record Low Vision Devices You can record the recommended low vision devices, acuity achieved, and skills for the patient on the LV Devices - OPT template. Summarize the Low Vision Exam Ophthalmic providers can write a brief summary of the exam, rehab notes, and education for low vision patients in the Summarization of Data panel. Update a Low Vision Device Entry
On the LV Devices - OPT template, expand the Devices panel. Select the device to modify in the grid. The device details fill the corresponding fields. Update the device details. Click Update. Parent topic: Record Low Vision Devices

Remove a Low Vision Device Entry
On the LV Devices - OPT template, expand the Devices panel. Select the device in the grid. The device details fill the corresponding fields. Click Remove. The device is removed from the grid. Parent topic: Record Low Vision Devices

Enter Visual Acuity Findings
To document visual acuity, you can select OD and OS values and modifiers, as appropriate. The system moves you to the next field as you document each parameter. For example, after you document the OD Dva sc and modifier, the picklist opens for OS Dva sc. In the Visual Acuity panel, click Add. The Oph Eyetech template opens. Select the OD value for Dva sc. The modifier pick list opens. Select a modifier from the list or click the blank row at the top of the list if no value is needed. The system moves to the OS value for Dva sc. Repeat the steps above for each parameter. Enter the Correction Type and Vision Method. Enter the Comments for OD, OS, or OU. Click Add. The documented findings are uploaded to the corresponding visual acuity grids Edit Visual Acuity Findings Remove Visual Acuity Findings You can only remove the visual acuity findings from the current encounter. Record Low Vision Treatment History
In the Low Vision Treatment History panel, select Yes or No for the History of Use of Low Vision Devices. In the Current Devices field, enter the current low vision device that the patient is using. Select Yes or No for Prior Low Vision Evaluation. If Yes, enter the date in the When field and By Whom. Enter the Recommendation for Additional Referrals. Parent topic: Low Vision History Template
