Record Low Vision Devices

Expand the Devices panel. Enter the Date the device was recommended to the patient and the Name of the device. Enter visual acuity in the VA field. Click VA Type to select from the following: Distance Near Occupational Intermediate Enter the Acuity Achieved, Skills, and Comments. Select Yes or No for Recommended to the Agency/Purchase. Mark a Low Vision Goal as Achieved

On the LV Devices - OPT template, expand the Goal panel. Select the goal in the grid. The goal details fill the corresponding fields. Modify Low Vision Goals

Remove a Low Vision Goal

Low Vision Devices

You can document and create a comprehensive, individual rehab plan for the low vision patient based on the patient's visual abilities, goals, and devices on the LV Devices - OPT template. Goals and devices can be monitored and updated in future encounters. Create Goals for the Low Vision Patient You can create custom goals for the low vision patient on the LV Devices - OPT template. Record Low Vision Devices You can record the recommended low vision devices, acuity achieved, and skills for the patient on the LV Devices - OPT template. Summarize the Low Vision Exam Ophthalmic providers can write a brief summary of the exam, rehab notes, and education for low vision patients in the Summarization of Data panel. Low Vision Exam Visit

A new visit type, Low Vision Exam, enables Optometry practices to document the patient's low vision history, the complete low vision exam, and recommended low vision devices. When you select the Optometry specialty and the Low Vision Exam visit type, a seven-tab workflow opens and includes the following low vision-specific tabs: LV History LV Exam LV Devices After you have finished documenting the low vision exam details, you can complete the encounter documentation with the standard workflows for assessment and plan, submitting charges, and patient checkout.
