Enter the Current Vision Prescription

Document Low Vision History

On the LV History - OPT template, go to the Low Vision History panel. If someone accompanies the patient, enter the details in the following fields: Accompanied by Relationship Enter Gradual or Sudden for the Type of Vision Loss. Enter No, Yes, or Unsure for the following: Fluctuations in Vision Issue with Glare Indoors Issue with Glare Outdoors Issue with Light/Dark Adaptation Current Sun Lenses Helpful Eye Pain/Discomfort Use of Eye Medication Enter a Description of Eye Condition. Click the date field to enter the Initial Onset of low vision. Select an option for the Preferred Eye. Enter the patient's living situation in the Current Living Situation field. Document Additional Notes

Enter additional notes in the Additional Notes panel. Click Close at the top of the template or select the LV Exam tab. Additional notes are added to the grid. Parent topic: Low Vision History Template

Record Eye Dilation

Dilation and other medications (eye drops) administered to the patient can be documented on the Dilation/Other Medications panel. Expand the Dilation/Other Medications panel. In the Dilation section, enter the medication used for dilation in the Dilated field. The patient was not dilated check box is disabled. Click the Eye field and select OD, OS, or OU. The system fills the Time of dilation and By fields with the current system time and name of the current user. If applicable, click the Patient and/or parent/guardian was advised of all side effects associated with dilation check box. Click Add. The dilation information is added to the grid. Low Vision History Template

You can record and view a detailed history for patients with low vision on the Low Vision History - OPT template. The Historic HPI, Impression/Plan Summary, Goal Summary, and Devices Summary panels contain grids that display corresponding historical findings for return patients. In addition, you can document the following: Dilations and other medications Current optical prescription Social history Low vision treatment history Visual Concerns Document Low Vision History The Low Vision History panel on the LV History - OPT enables you to record the history of the patient's vision issues. Record Eye Dilation You can record the medication used for dilation and other medications administered to the patient during the visit. Document the Reason a Patient was Not Dilated You can record the reason why a patient was not dilated in the Dilation/Other Medication panel. Document Other Medications (Eye Drops) Administered to a Patient How to document eye drop medications administered to a patient.

Record Low Vision Devices

Expand the Devices panel. Enter the Date the device was recommended to the patient and the Name of the device. Enter visual acuity in the VA field. Click VA Type to select from the following: Distance Near Occupational Intermediate Enter the Acuity Achieved, Skills, and Comments. Select Yes or No for Recommended to the Agency/Purchase.
