Record Low Vision Treatment History
In the Low Vision Treatment History panel, select Yes or No for the History of Use of Low Vision Devices. In the Current Devices field, enter the current low vision device that the patient is using. Select Yes or No for Prior Low Vision Evaluation. If Yes, enter the date in the When field and By Whom. Enter the Recommendation for Additional Referrals. Parent topic: Low Vision History Template

Low Vision History Template
You can record and view a detailed history for patients with low vision on the Low Vision History - OPT template. The Historic HPI, Impression/Plan Summary, Goal Summary, and Devices Summary panels contain grids that display corresponding historical findings for return patients. In addition, you can document the following: Dilations and other medications Current optical prescription Social history Low vision treatment history Visual Concerns Document Low Vision History The Low Vision History panel on the LV History - OPT enables you to record the history of the patient's vision issues. Record Eye Dilation You can record the medication used for dilation and other medications administered to the patient during the visit. Document the Reason a Patient was Not Dilated You can record the reason why a patient was not dilated in the Dilation/Other Medication panel. Document Other Medications (Eye Drops) Administered to a Patient How to document eye drop medications administered to a patient.

Low Vision Exam Visit
A new visit type, Low Vision Exam, enables Optometry practices to document the patient's low vision history, the complete low vision exam, and recommended low vision devices. When you select the Optometry specialty and the Low Vision Exam visit type, a seven-tab workflow opens and includes the following low vision-specific tabs: LV History LV Exam LV Devices After you have finished documenting the low vision exam details, you can complete the encounter documentation with the standard workflows for assessment and plan, submitting charges, and patient checkout. Low Vision Exam
The provider can document the exam for a low vision visit on the LV Exam - OPT template. You can review the testing performed by the technician and perform additional exams as needed. Enter Visual Acuity Findings Document an External Exam You can document the external exam of an eye in the External Exam panel. Document a Slit Lamp Exam Document a Posterior Exam Enter Cover Test Findings Enter Nystagmus Findings You can record findings associated with nystagmus in the Nystagmus panel. Record Color Vision Findings You can record color vision deficiencies in the Color Vision panel. Document Corneal Light Reflex Test Results You can enter your findings from the Corneal Light Reflex test in the Corneal Light Reflex Test panel.
