The provider can document the exam for a low vision visit on the LV Exam - OPT template. You can review the testing performed by the technician and perform additional exams as needed. Low Vision Devices

You can document and create a comprehensive, individual rehab plan for the low vision patient based on the patient's visual abilities, goals, and devices on the LV Devices - OPT template. Goals and devices can be monitored and updated in future encounters. Create Goals for the Low Vision Patient You can create custom goals for the low vision patient on the LV Devices - OPT template. Record Low Vision Devices You can record the recommended low vision devices, acuity achieved, and skills for the patient on the LV Devices - OPT template. Summarize the Low Vision Exam Ophthalmic providers can write a brief summary of the exam, rehab notes, and education for low vision patients in the Summarization of Data panel. Document Low Vision History

On the LV History - OPT template, go to the Low Vision History panel. If someone accompanies the patient, enter the details in the following fields: Accompanied by Relationship Enter Gradual or Sudden for the Type of Vision Loss. Enter No, Yes, or Unsure for the following: Fluctuations in Vision Issue with Glare Indoors Issue with Glare Outdoors Issue with Light/Dark Adaptation Current Sun Lenses Helpful Eye Pain/Discomfort Use of Eye Medication Enter a Description of Eye Condition. Click the date field to enter the Initial Onset of low vision. Select an option for the Preferred Eye. Enter the patient's living situation in the Current Living Situation field. Low Vision Exam Visit

A new visit type, Low Vision Exam, enables Optometry practices to document the patient's low vision history, the complete low vision exam, and recommended low vision devices. When you select the Optometry specialty and the Low Vision Exam visit type, a seven-tab workflow opens and includes the following low vision-specific tabs: LV History LV Exam LV Devices After you have finished documenting the low vision exam details, you can complete the encounter documentation with the standard workflows for assessment and plan, submitting charges, and patient checkout. Document Other Medications (Eye Drops) Administered to a Patient

You can document other medications administered to the patient that are not prescribed to the patient in the Other medications section. Medications entered here do not appear in the Medication module. In the Other medications section, do one of the following: Click the Name field and select the eye drops administered from the list. To enter drops not listed, select the top blank line and type the desired value. Select the designated eye in the Eye field. The Time of dilation and By fields display the current system time and the current user name Enter comments in the Comment field. Click Add.
