.
Click the
Reason patient not dilated
field and do one of the following:
Select a reason from the list.
Select the blank line at the top of the list to type the reason.
Click the
Eye
field and select
OD
,
OS
, or
OU
.
The
By
field displays the current user name.
Click
Add
.
The information appears in the grid.

Record Eye Dilation
Dilation and other medications (eye drops) administered to the patient can be documented on the Dilation/Other Medications panel. Expand the Dilation/Other Medications panel. In the Dilation section, enter the medication used for dilation in the Dilated field. The patient was not dilated check box is disabled. Click the Eye field and select OD, OS, or OU. The system fills the Time of dilation and By fields with the current system time and name of the current user. If applicable, click the Patient and/or parent/guardian was advised of all side effects associated with dilation check box. Click Add. The dilation information is added to the grid. Document Other Medications (Eye Drops) Administered to a Patient
You can document other medications administered to the patient that are not prescribed to the patient in the Other medications section. Medications entered here do not appear in the Medication module. In the Other medications section, do one of the following: Click the Name field and select the eye drops administered from the list. To enter drops not listed, select the top blank line and type the desired value. Select the designated eye in the Eye field. The Time of dilation and By fields display the current system time and the current user name Enter comments in the Comment field. Click Add. Access Patient Dilation/Other Medications
Patient dilation and other eye drop medications can be documented directly on the Dilation/Other Medications panel. To access Patient Dilation/Other Medications, do one of the following: From the Prelim Exam – OPH template, expand the Patient Dilation/Other Medications panel. From the Provider – OPH template, click the Dilation link to open the Dilation/Other Medications - OPH template. Record Low Vision Treatment History
In the Low Vision Treatment History panel, select Yes or No for the History of Use of Low Vision Devices. In the Current Devices field, enter the current low vision device that the patient is using. Select Yes or No for Prior Low Vision Evaluation. If Yes, enter the date in the When field and By Whom. Enter the Recommendation for Additional Referrals. Parent topic: Low Vision History Template

Document Low Vision History
On the LV History - OPT template, go to the Low Vision History panel. If someone accompanies the patient, enter the details in the following fields: Accompanied by Relationship Enter Gradual or Sudden for the Type of Vision Loss. Enter No, Yes, or Unsure for the following: Fluctuations in Vision Issue with Glare Indoors Issue with Glare Outdoors Issue with Light/Dark Adaptation Current Sun Lenses Helpful Eye Pain/Discomfort Use of Eye Medication Enter a Description of Eye Condition. Click the date field to enter the Initial Onset of low vision. Select an option for the Preferred Eye. Enter the patient's living situation in the Current Living Situation field. You can document dilation and (eye drops) administered in the Dilation/Other Medications panel.
