Patient's History Summary

The History Summary panel on the Intake-OPH template enables you to document a patient's past medical or surgical history, interim, social, family, diagnostic study, and glucose monitoring history. It also displays the patient history documented by the Intake staff on the Histories template. You can also add diagnostic studies with a documented Result or Interpretation to the grid in the Reason for Visit panel by clicking the Add to HPI Grid button. The grid includes a list of categories in the left pane to select and view the documented data. Complete the Ophthalmology Provider Workflow

For most office visit encounters in general Ophthalmology, the following is a basic outline of the workflow for the provider portion of the visit. To review historical information of the patient, go to the Home Page – OPH template. Go to the Provider – OPH template to document the provider's portion of the encounter. Note: For certain encounters, there are specific single-page visit templates, such as Postoperative Exam – OPH. Select the Provider Summary link to review patient information for today's encounter. Select the Eye Summary link to view current and historical patient findings in a grid. Expand the General panel to review general patient information, the Health Monitor, and Internal Physician Notes. Expand the Orders panel to review, update, and add orders. Expand the Medications panel to review or update medications and perform medication reconciliation, if appropriate. Document and Sign-off HPI as an Ophthalmic Provider

Ophthalmic providers document HPI information on the Generic HPI - OPH template in the Provider HPI Details section. When you open the Generic HPI - OPH template you can document additional reasons for visit. All chief complaints selected as reasons for visit, except retina, are documented on the Generic HPI - OPH template. If previously selected, the chief complaint retina appears in the Chief complaint details grid, but you can only update the HPI information on the Retina History of Present Illness - OPH template. Note: The Chief complaint details grid displays only the technician’s documentation. The final HPI grid shows the provider’s updates and notes. On the Reason for Visit panel, do one of the following to open the Generic HPI - OPH template. Select any chief complaint, except retina, in the Reason for Visit/History of Present Illness grid. Filter the Historical Diagnostic Tests - OPH Grids

You can filter the diagnostic tests and test history on the Historical Diagnostic Tests - OPH template. To filter the tests displayed in the Diagnostic Test History grid, select one of the following Show options: Ordered Obtained Completed All tests The Diagnostic Test History grid displays tests based on the selected option. To filter the Additional Test History grid, click View and select one of the following: All Amsler grid Aniseikonia test Exophthalmometry Glare/Contrast Horizontal corneal diameter Ishihara color test PAM/RAM acuity meter Schirmer w/ Anesth Schirmer w/o Anesth Stereo acuity Stereo vision Worth 4 dot test The grid displays test history based on the selected View option. Ophthalmology Provider Workflow

Provider workflow may begin at the Home Page - OPH template, where you can review patient information. The provider workflow for documenting a patient exam continues on the Provider - OPH template. Providers can review tests performed by the technician, document the physical exam, and record tests performed during the provider's portion of the encounter. Add a Diagnostic Study to the HPI Grid

You can add a diagnostic study that is reviewed during the encounter to the history of present illness (HPI). Open one of the following: History Summary panel Historical Diagnostic Tests - OPH template Diagnostic History Entry template Select a diagnostic study with a documented Result or Interpretation.The Add to HPI Grid button is enabled. Note: The Add to HPI Grid button is only enabled if the selected study has a documented Result or Interpretation.
