Document and Sign-off HPI as an Ophthalmic Provider

Ophthalmic providers document HPI information on the Generic HPI - OPH template in the Provider HPI Details section. When you open the Generic HPI - OPH template you can document additional reasons for visit.

All chief complaints selected as reasons for visit, except retina, are documented on the Generic HPI - OPH template. If previously selected, the chief complaint retina appears in the Chief complaint details grid, but you can only update the HPI information on the Retina History of Present Illness - OPH template.

  1. On the Reason for Visit panel, do one of the following to open the Generic HPI - OPH template.
    • Select any chief complaint, except retina, in the Reason for Visit/History of Present Illness grid.
    • Select any chief complaint, except retina, from the left pane.
    The HPI Generic OPH template opens, displaying the chief complaints and HPI information previously documented by the technician.

  2. Select a chief complaint in the Chief complaints details grid.
    The chief complaint appears in the Concern(s) field and previously documented details appear in the Provider HPI Details section and the Technician Conversation section. The current user name appears in the HPI details completed by field.
  3. You can review the HPI details in the Provider HPI Details section. You can make changes or additions to HPI information by selecting from picklists, entering text, or using My Phrases.
    1. To enter HPI information through picklist selections, start by clicking the Location detail link. Click a detail link and select details from the list that opens. For lists where you can make multiple sections, click Close to exit the list after you have made all selections.
      A summary statement builds in the large HPI details field as you document HPI details. The corresponding check box is selected for each area that was documented.
    2. You can type additional information in the HPI details field.
    3. You can enter additional details using the My Phrases link.
  4. After reviewing and updating HPI details for a chief complaint, in the Provider HPI Details section, click Update.
    The HPI is complete and signed-off and appears in the Reason for Visit grid and on the HOPI Notes template.
  5. Repeat the steps above for all remaining chief complaints listed in the Chief complaint details grid.
  6. Click Close.
    The Reasons for Visit panel displays the completed HPI information.
