Reason for Visit

You can document the reason for visit as described by the patient on the Reason for Visit panel. In NextGen® Ophthalmic Suite, the retina reason for visit opens the Retina History of Present Illness - OPH template. All other reasons for visit open the Generic HPI - OPH template. Once opened, you can document multiple reasons for visits on the Generic HPI - OPH template. The Diagnostics button opens the Historical Diagnostic Tests - OPH template, where you can review historical diagnostic tests and add them to the HPI grid. You can configure the Reason for Visit panel on the Framework Content system template. Complete the Ophthalmology Provider Workflow

For most office visit encounters in general Ophthalmology, the following is a basic outline of the workflow for the provider portion of the visit. To review historical information of the patient, go to the Home Page – OPH template. Go to the Provider – OPH template to document the provider's portion of the encounter. Note: For certain encounters, there are specific single-page visit templates, such as Postoperative Exam – OPH. Select the Provider Summary link to review patient information for today's encounter. Select the Eye Summary link to view current and historical patient findings in a grid. Expand the General panel to review general patient information, the Health Monitor, and Internal Physician Notes. Expand the Orders panel to review, update, and add orders. Expand the Medications panel to review or update medications and perform medication reconciliation, if appropriate. Historical Diagnostic Tests

You can view historical data for diagnostic tests for the patient on the Historical Diagnostic Tests - OPH template, which you can access from the left navigation pane. The Add to HPI button adds the selected diagnostic study with an Interpretation or Result to the grid in the Reason for Visit panel. Providers can also add entries to the Diagnostic Test History grid. Add a Chief Complaint on the HPI Generic OPH Template as a Provider

To add a chief complaint on the HPI Generic OPH template, click the Concern field. Select any complaint, except retina, from the list on the Reason for Visit window and click Save & Close. The selected complaint appears in the Chief complaint details grid. Select the newly entered chief complaint in the grid. In the Provider HPI Details section, choose the desired options below: To enter HPI information using the HPI links, starting with the Location detail link, click a detail link and select details from the list that opens. A summary statement builds in the large HPI details field as you document HPI details, and the system selects the corresponding check box. At any point, you can type additional information in the HPI details field. You can enter additional details using the My Phrases link. Click Update. Edit a Chief Complaint After Provider Sign Off

Before you begin A chief complaint must be documented in the Chief complaint details grid. After the provider has signed off on the ophthalmic chief complaint, you must make additional edits in the Provider HPI Details section of the HPI Generic OPH template. To edit a chief complaint on the HPI Generic OPH template, highlight the row in the Chief complaint details grid. The highlighted chief complaint appears in the Concern(s) field. Note: If you select retina from the Chief complaints details grid, the system displays an alert above the grid and changes the fields on the HPI Generic OPH template to read-only In the Provider HPI Details section, make changes to the chief complaint. Enter Ophthalmic Chief Complaints

Technicians can document HPI details in the Technician Conversation section of the Generic HPI - OPH template for ophthalmic complaints except for retina. Note: You must document the retina chief complaint on the Retina History of Present Illness - OPH template. On the Reason for Visit panel, do one of the following to open the Generic HPI - OPH template: Click a chief complaint in the left pane. Select a chief complaint in the Reason for Visit/History of Present Illness grid.
