Accessing Population Health Sync from NextGen® Enterprise EHR

If your enterprise is enrolled in Population Health Sync, you can access data from it directly within NextGen® Enterprise EHR as well as from the NextGen® Population Health and Analytics Solutions (formerly EagleDream Health) website. You can view available gaps-in-care and risk score data for a selected patient from the Patient Information Card (PIC). Population Health Sync extracts patient data for analysis from NextGen® Enterprise EHR every 48 hours by default. An internal Application Program Interface (API) passes the analytical data from Population Health Sync to NextGen® Enterprise EHR for display. Population Health Sync works at the enterprise level. The same data for a patient is provided to all practices within your enterprise that are configured for Population Health Sync reporting. Enterprise Preferences for Population Health Sync

Note: In Enterprise Preferences, you can set up both Population Health Sync and Clinical Quality Reporting, but you can select only one of these reporting features at a time to view in NextGen® Enterprise EHR. The settings are retained if you switch from one reporting option to the other. To configure Enterprise Preferences for Population Health Sync reporting, you must initially enter global system settings that apply across enterprises and then enter credential settings for each participating practice in your enterprise. Gaps-in-Care Indicators

Four color-coded triangles in the Population Health Sync section indicate the gaps in care for a patient. When you hover over a gaps-in-care triangle, a brief description appears: Red triangle. Not Meeting Goal. Orange triangle. Past Due. Yellow triangle Near Due. Green triangle Meeting Goal. Each triangle displays the number of gaps in care for the selected patient. Based on the type and count of gaps in care, you can decide whether to select a triangle to open the patient profile for further review. For example, if the patient has no gaps in care marked Past Due or Not Meeting Goal, you may not need more information. If the patient does not have Population Health Sync data, the gaps-in-care triangles in the Population Health Sync section are grayed out and disabled. View a Patient's Gaps in Care

If a patient has gaps in care, you can view the details on the Patient Profile from the NextGen Population Health and Analytics Solutions website. You cannot make any changes to the information. Open NextGen® Enterprise EHR. Perform a patient search, and open the chart for a patient with Population Health Sync data. The gaps in care indicators appear in the Population Health Sync section on the Patient Information Card. Select a gaps in care triangle. The Patient Profile opens with the patient information tab in view. The patient's name appears on the tab. You can scroll down to see the Gaps in Care section. Note: If you are running NextGen® Enterprise version 5.9 or earlier, the first tab of the Patient Profile opens in your default browser. The section displays the same number of gaps in care as the Population Health Sync section on the Patient Information Card. It lists each gap in care, the date last done, the due date, and status. View a Patient's Risk Report

If a patient has risk score data available, you can display a detailed Risk report from the NextGen Population Health and Analytics Solutions website. You cannot make any changes to the report. Open NextGen® Enterprise EHR. Perform a patient search, and open the chart for a patient with Population Health Sync data. The risk score indicator appears in the Population Health Sync section on the Patient Information Card (PIC). Select the Risk Score circle . The Patient Profile window opens, with the Risk Report tab in view. Note: If you are running NextGen® Enterprise EHR version 5.9 or earlier, the Risk Report tab opens in your default browser. The Risk Report contains three main sections: The Risk Overview displays the same risk scores shown when you hover over the Risk Score circle in the Patient Information Card.
