Gaps-in-Care Indicators

Four color-coded triangles in the Population Health Sync section indicate the gaps in care for a patient.

When you hover over a gaps-in-care triangle, a brief description appears:
  • Red triangle. Not Meeting Goal.
  • Orange triangle. Past Due.
  • Yellow triangle Near Due.
  • Green triangle Meeting Goal.

Each triangle displays the number of gaps in care for the selected patient.

Based on the type and count of gaps in care, you can decide whether to select a triangle to open the patient profile for further review. For example, if the patient has no gaps in care marked Past Due or Not Meeting Goal, you may not need more information.

If the patient does not have Population Health Sync data, the gaps-in-care triangles in the Population Health Sync section are grayed out and disabled. If you hover over the triangles, a message appears indicating that no patient data exists.

Gaps-in-Care Indicators
