View a Patient's Risk Report

If a patient has risk score data available, you can display a detailed Risk report from the NextGen Population Health and Analytics Solutions website. You cannot make any changes to the report. Open NextGen® Enterprise EHR. Perform a patient search, and open the chart for a patient with Population Health Sync data. The risk score indicator appears in the Population Health Sync section on the Patient Information Card (PIC). Select the Risk Score circle . The Patient Profile window opens, with the Risk Report tab in view. Note: If you are running NextGen® Enterprise EHR version 5.9 or earlier, the Risk Report tab opens in your default browser. The Risk Report contains three main sections: The Risk Overview displays the same risk scores shown when you hover over the Risk Score circle in the Patient Information Card. For definitions of each risk type, you can select the information icon next to Risk Overview. The ACG Patient Report Overview provides the patient's Adjusted Clinical Groups (

Gaps-in-Care Indicators

Four color-coded triangles in the Population Health Sync section indicate the gaps in care for a patient. When you hover over a gaps-in-care triangle, a brief description appears: Red triangle. Not Meeting Goal. Orange triangle. Past Due. Yellow triangle Near Due. Green triangle Meeting Goal. Each triangle displays the number of gaps in care for the selected patient. Based on the type and count of gaps in care, you can decide whether to select a triangle to open the patient profile for further review. For example, if the patient has no gaps in care marked Past Due or Not Meeting Goal, you may not need more information. If the patient does not have Population Health Sync data, the gaps-in-care triangles in the Population Health Sync section are grayed out and disabled. Population Health Sync on the Patient Information Card

If you have configured Enterprise Preferences for Population Health Sync reporting, a section with indicators for gaps in care and the risk score appears on the Patient Information Card. The indicator colors and counts enable you to quickly assess the information available and then, access the patient profile for more details. If any invalid settings were made in Enterprise Preferences, the Population Health Sync section gray out and disables. If you hover over the section, a message appears indicating that connection with the server is unsuccessful. Note: If you choose to install Holon application, Population Health Sync do not appear on the Patient Information Card of NextGen® Enterprise EHR, but it continues to appear on the Care Opportunities tab of NextGen Care® Population Management Hub. Viewing Patient Insights

If your patient in the Primary Care, Federally Qualified Health Centers (FQHC), or Behavioral Health specialty, synchronized with Population Health System (PHS) has clinical gaps in care, you can assess and resolve the Hierarchical Condition Categories (HCC) diagnoses in the Patient Insights section in Patient Chart. You can provide the patient the necessary care and help your practice obtain reimbursement depending on the patient's insurance. The gaps count of upcoming and overdue items, and the Adjusted Clinical Groups (ACG) risk score for the patient appear on the header. You can select the Patient Insights section to view the following details: Risk score overview In Risk Score, the ACG, Atherosclerotic Cardiovascular Disease (ASCVD), and HCC scores appear for patients having clinic visits in the last twelve months.
