Risk Score Indicator
A color-coded circle in the Population Health Sync section indicates the patient's risk score.
Risk colors, scores, and definitions are as follows:
- 0 (Blue). Non Users.
- 1 (Green). Healthy.
- 2 (Yellow). Low.
- 3 (Orange). Moderate.
- 4 (Red). High.
- 5 (Maroon). Very High.
When you hover over the Risk Score circle, the ACG Rub score appears along with a Risk Overview that includes the HCC Risk score and the ASCVD Risk score. To view additional information, you can select the circle to open the Risk Report tab on the Patient Profile window.
If the patient does not have Population Health Sync data, the Risk Score circle in the Population Health Sync section is grayed out and disabled. If you hover over the circle, a message appears indicating that no patient data exists.
