If a patient has gaps in care, you can view the details on the Patient Profile from the NextGen Population Health and Analytics Solutions website. You cannot make any changes to the information.
- Open NextGen® Enterprise EHR.
- Perform a patient search, and open the chart for a patient with Population Health Sync data.
The gaps in care indicators appear in the Population Health Sync section on the Patient Information Card.
- Select a gaps in care triangle.
The
Patient Profile opens with the patient information tab in view. The patient's name appears on the tab.
You can scroll down to see the
Gaps in Care section.
Note: If you are running NextGen® Enterprise version 5.9 or earlier, the first tab of the Patient Profile opens in your default browser.
The section displays the same number of gaps in care as the Population Health Sync section on the Patient Information Card. It lists each gap in care, the date last done, the due date, and status. You can filter the gaps in care across all the initiatives or a selected initiative that the selected patient qualifies for. You can view the gaps in care information by Status or Name. Actions for gaps in care are disabled.

icon.
You can select other tabs to review, but you cannot perform any other actions in the Patient Profile.
