Problem List

You can record, modify and manage problems in the Problems List on the Histories-Cardiology template. The Problem List section enables you to do the following. Record, modify and retrieve patient's clinical problem list. Document patient's chronic conditions and secondary problems. View the problems from the past encounters. Update the problem details. Set Clinical Status or Resolved Date for Problems

You can set a clinical status or resolved date for a problem on the Problems panel on the Histories-Cardiology template. Open NextGen® Enterprise EHR. Open the Histories - Cardiology template. Click the Problems panel. In the Problems List section, select a problem to update the status. The details of the selected problem have been displayed in the fields. Problem List To update the clinical status or resolved date, do one of the following. To update the clinical status, click Clinical Status, select a status, and click OK. Document Cardiac Problems

The Problems panel enables you to document, search for, and select the chronic and secondary conditions of a patient's problems. Open NextGen® Enterprise EHR. Open the Histories - Cardiology template. Click the Problems panel. In the Cardiac Problems List, click a link to open the template. The template associated with the link opens. Enter a date in the Date PopUp template and click Accept. In the Comments section, enter the history and details of the problem. Note: In case the patient has no problems, this field does not need to be added. Card CH Valvular Click Accept. The resulting window opens with all problems listed on the Problem List. Do one of the following on the problem list. Choose to add the current problem as a new problem. Merge it into an existing problem as a problem note. If you want to add a new problem, click Add new problem, and select the appropriate problem description from the resulting window. Click Select. The problem has been added to the Cardiac Problem List

Add Cardiac Problems to Problem List

You can add a cardiac problem to the Problems List on the Histories- Cardiology template. Open NextGen® Enterprise EHR. Open the Histories- Cardiology template. Click the Problem List and click Add/Update. A diagnosis search window opens. Enter a problem in the diagnosis search window and click Search. Select a problem from the list and click Select. The Fully Specified SNOMED Description, Problem Description, ICD Description, and ICD Code have been added and displayed in the Problem List panel. For all new problems, Mark as Chronic has been selected by default. Select clinical status, side, and site in Problem List. Click Onset Date and select the date of onset. Enter an approximate date if the exact onset date is unknown. To select the date when the problem was resolved, click Resolved Date. To mark the problem as a secondary condition, select Secondary Condition if the problem is not chronic. To add the selected problem to the tracked problem list, select Marked as Tracked Problem.

You can update a cardiac problem in Problems List on the Histories template. Open NextGen® Enterprise EHR. Open the Histories- Cardiology template. Click the Problem List. Add Problems in the Problem List

You can add a problem to the problem list of a patient. Open NextGen® Enterprise EHR. Open the Histories template. In the Problem List panel, select Add/Update. Enter a problem in the diagnosis search window, and select Search. Select the problem from the list of the search result, and select Select. Select the applicable ICD code, and select Select. The application automatically fills the associated values in the Fully Specified SNOMED Description, Problem Description, ICD Description, and ICD Code fields in the Problem List panel. Note: For all new problems, the Mark as Chronic check box is selected by default. In Clinical Status, select the status of the problem. In Onset Date, enter the date of onset. In Resolved Date, enter the date when the selected problem is resolved. If the problem is not chronic, to mark the problem as a secondary condition, select Secondary Condition.
