Add Cardiac Problems to Problem List

You can add a cardiac problem to the Problems List on the Histories- Cardiology template.

  1. Open NextGen® Enterprise EHR.
  2. Open the Histories- Cardiology template.
  3. Click the Problem List and click Add/Update.
    A diagnosis search window opens.
  4. Enter a problem in the diagnosis search window and click Search.
  5. Select a problem from the list and click Select.
    The Fully Specified SNOMED Description, Problem Description, ICD Description, and ICD Code have been added and displayed in the Problem List panel. For all new problems, Mark as Chronic has been selected by default.
  6. Select clinical status, side, and site in Problem List.
  7. Click Onset Date and select the date of onset. Enter an approximate date if the exact onset date is unknown.
  8. To select the date when the problem was resolved, click Resolved Date.
  9. To mark the problem as a secondary condition, select Secondary Condition if the problem is not chronic.
  10. To add the selected problem to the tracked problem list, select Marked as Tracked Problem.
  11. Click Responsible Provider and select a physician's name.
  12. To move the problem to today's assessment list, select Move to Today’s Assessments.
  13. Select Move to Cardiac Problem List to move the problem to Cardiac Problem List.
  14. To enter notes specific to the problem, use Problem Notes.
  15. To add the problem to the problem list, click Add/Update.
