Open Problem List
You can access the problem list to document and view a patient's problem history. Open NextGen® Enterprise EHR. Select a patient record. To access the Problem List panel, do one of the following: In the Histories template, expand the Problem List panel. In the Home Page template, expand the Problem List panel. Note: You can use Problems in the quick button panel to access the Problems module. Add Problems in the Problem List You can add a problem to the problem list of a patient. Add Cardiac Problems to Problem List
You can add a cardiac problem to the Problems List on the Histories- Cardiology template. Open NextGen® Enterprise EHR. Open the Histories- Cardiology template. Click the Problem List and click Add/Update. A diagnosis search window opens. Enter a problem in the diagnosis search window and click Search. Select a problem from the list and click Select. The Fully Specified SNOMED Description, Problem Description, ICD Description, and ICD Code have been added and displayed in the Problem List panel. For all new problems, Mark as Chronic has been selected by default. Select clinical status, side, and site in Problem List. Click Onset Date and select the date of onset. Enter an approximate date if the exact onset date is unknown. To select the date when the problem was resolved, click Resolved Date. To mark the problem as a secondary condition, select Secondary Condition if the problem is not chronic. To add the selected problem to the tracked problem list, select Marked as Tracked Problem.

Add Problem to the Problem List
To open the Problems window, do one of the following: In the Problem List panel, click Add/Update Click Problems in the patient information bar. The Problems window opens. Enter a problem in the diagnosis search window and click Search. Highlight the problem from the list and click Select. The system fills the associated values in the Fully Specified SNOMED® Description, Problem Description, ICD Description and ICD Code fields in the Problem List panel. For all new problems, Mark as Chronic is selected by default. Enter the Clinical Status. Enter the Onset Date. Enter the Resolved Date. If the problem is not chronic, do one of the following: Select Secondary Condition to mark the problem as a secondary condition. Select Marked as Tracked Problem to add the selected problem to the tracked problem list. To add the problem to the assessment list, select Add to Today’s Assessments. In Problem Notes, enter notes specific to the problem. Click Add/Update to add the problem to the problem lis

Add a Problem in the Problem List
You can add a problem to the problem list of a patient. Open the Histories template. In the Problem List panel, click Add/Update. The Problems template opens. Click Add Problem. A search window open. Enter the full or partial name of the problem in the field at the top of the search window. Click Search. A list of search records displays under the Description column. Select a problem and click Select. Note: To add the selected problem to the tracked problem list, select the Add to My Tracked Problems check box. The selected record displays in the Problem List module. From the Problem Status list, select the status of the problem. If the selected problem is chronic, select the Chronic check box, otherwise select the Secondary Condition check box. Note: The Onset Date field displays the current date. You can enter an approximate date if the exact onset date is unknown. In the Resolved Date field, enter the date when the selected problem was resolved. Click Accept. The problem is grouped

Adding a Tracked Diagnosis from the Patient's Problem List to the ICD Billing List
You can add a diagnosis from a patient's problem list to ICD billing list for that patient. A chronic or tracked condition diagnosis from the patient's problem list can be added to the ICD billing list. This causes the diagnosis to be associated with an ICD billing code, and enables it to be associated with a care guideline. Open NextGen® Enterprise EHR. Select the Care Guidelines link on any of the main template. The Care Guidelines template opens. Select Add/Update in the Care Guidelines panel. The Activate Care Guidelines template opens. Expand the Problem List panel, do the following: Select the Show chronic check box to display chronic problems only. Select the Show my tracked problems check box to display only those diagnoses that have been assigned to the patient’s tracked problems list. Select the No active problem check box, to indicate that the patient currently does not have an active problem. Problem List Note: To open the Problems module, select Add or Edit. To reload the
