Add Problem to the Problem List

  1. To open the Problems window, do one of the following:
    • In the Problem List panel, click Add/Update
    • Click Problems in the patient information bar.
    The Problems window opens.
  2. Enter a problem in the diagnosis search window and click Search.
  3. Highlight the problem from the list and click Select.
    The system fills the associated values in the Fully Specified SNOMED® Description, Problem Description, ICD Description and ICD Code fields in the Problem List panel. For all new problems, Mark as Chronic is selected by default.
  4. Enter the Clinical Status.
  5. Enter the Onset Date.
  6. Enter the Resolved Date.
  7. If the problem is not chronic, do one of the following:
    • Select Secondary Condition to mark the problem as a secondary condition.
    • Select Marked as Tracked Problem to add the selected problem to the tracked problem list.
  8. To add the problem to the assessment list, select Add to Today’s Assessments.
  9. In Problem Notes, enter notes specific to the problem.
  10. Click Add/Update to add the problem to the problem list.
