Summary of Care Requests and Receipts

You can open the Transitioning Into Care template which records electronic Summary of Care requests and receipts as reportable fields on the template.

The Transitioning Into Care template indicates whether a summary of care records for a specific patient are requested electronically and if any summary of care records are received. You can also document the completion of a Summary of Care request without a document returned.

The Transitioning into Care link is available in the General panel of the templates. The information icon next to the Transitioning Into Care template provides basic template instructions. You can use the Clinical Reconciliation link to open the Clinical Reconciliation module.

You can access the actual Summary of Care documents through a patient’s chart. These documents are available on the Categories tab.
