Remove a Medication from the Oncology Medication History Grid

You can only remove records created during the current encounter. Access the Onc Treatment Summary template and scroll to the Oncology Medication History panel. Locate and select the medication to be removed on the Oncology Medication History grid. The system updates the fields with data from the selected record. Click Remove. Update a Medicaton on the Oncology Medication History Grid

You can only update a medication record entered for the current encounter. Access the Onc Treatment Summary template and scroll to the Oncology Medication History panel. Locate and select the medication to be updated on the Oncology Medication History grid. The system populates the fields with data from the selected record. Enter Cancer Data

Access the Onc Treatment Summary template and scroll to the Notes panel. Click the Type field and select Progress from the list. Type a progress note in the data entry area and click Add. The progress note appears in the grid. Click the Type field and select Radiation Oncology from the list. Type a radiation oncology narrative in the data entry area and click Add. The narrative appears in the grid. Scroll to the Problem List panel, click Add or Edit, and enter the following information: Problem Description Onset Date onc treatment summary template prob list Return to the Onc Treatment Summary template and select Tobacco Cessation from the left Navigation menu. The Tobacco Cessation template appears. Click the Tobacco Usage link. The Social History – Tobacco template appears. Enter the patient’s smoking use information and click OK. Access the Onc Treatment Summary Template

Open a patient encounter and select a Specialty of Oncology and a Visit Type of Office visit. Note: Ensure the patient has an appropriate cancer diagnosis code in the Billing ICD list. The diagnosis code must match a valid CRR Report Code. Document the condition in the patient’s Problem List module. Select Cancer Treatment Summary from the navigation bar. The Onc Treatment Summary template appears. Stage Cancer from the Onc Treatment Summary Template

The Onc Treatment Summary template provides the most comprehensive method of entering cancer staging and related data, including cancer medications, treatment notes and other data. For Cancer Registry Report, you must provide data on the following: Dx Date (Diagnosis Date) Cancer Type/Site Histology Behavior Laterality Body Site Diagnostic Confirmation Stage a Cancer Prior to staging cancer, you should enter the cancer diagnosis for the patient. If you do not, you can select a diagnosis during the procedure. Medications

You can document and manage the medication history and current medications of a patient. You can use the Medications panel to view the patient's current medications and medication history, enter additional prescriptions for the patient, and reconcile the medications listed with the actual active medications for the patient. You can use the No medications this encounter check box if there are no active medications for the patient. Medications Panel You can reconcile the medications for the patient using the Medications reconciled check box, and indicate that you have received the summary of care for the patient using the Summary of care received check box. You can select the Reconcile button to use the following: Medication Review to review the patient medication on the Medication Review template Electronic Reconciliation to electronically reconcile the medications on the Clinical Reconciliation template The documented medication records appear in rows.
