template and scroll to the

panel.
Click the

Type a progress note in the data entry area and click

Type a radiation oncology narrative in the data entry area and click
Add

The narrative appears in the grid.
Scroll to the
Problem List
panel, click
Add

Edit

Return to the
Onc Treatment Summary
template and select
Tobacco Cessation
from the left
Navigation
menu.
The
Tobacco Cessation
template appears.
Click the
Tobacco Usage
link.

Enter the patient’s smoking use information and click

Access the Onc Treatment Summary Template
Open a patient encounter and select a Specialty of Oncology and a Visit Type of Office visit. Note: Ensure the patient has an appropriate cancer diagnosis code in the Billing ICD list. The diagnosis code must match a valid CRR Report Code. Document the condition in the patient’s Problem List module. Select Cancer Treatment Summary from the navigation bar. The Onc Treatment Summary template appears. Onc Treatment Summary From the Onc Treatment Summary template, you can: Enter demographic information Add cancer staging data Add additional cancer staging details Modify previously entered cancer records Create a copy of a staging record Remove a cancer staging record Add cancer treatment notes Specify cancer-related medications Generate an Oncology Treatment Summary Parent topic: Cancer Registry Report Workflow Related tasks Record Patient Demographic Data

Remove Notes
Access the Onc Treatment Summary template and scroll to the Notes panel. Select the note to be removed in the Notes grid. The data entry fields are populated with the data from the selected note. Click Remove. Parent topic: Add Notes Related tasks Access the Onc Treatment Summary Template

Add Notes
Access the Onc Treatment Summary template and scroll to the Notes panel. Notes Click the Type field and select a note type from the list. Note: My Phrases functionality is keyed to the selected note type enabling you to define a separate set of My Phrases for each type of cancer note. For more information on managing and using the My Phrases functionality, go to NextGen Healthcare Success Community and download the latest Getting Started User Guide for NextGen Adaptive Content Engine. Type notations about the patient or the patient's condition or disease into the Note field. Note: Use the Clear button to clear the data entry fields of all data. Click Add. The system adds the note to the Notes grid. Note: The Notes grid only shows Notes added during the current encounter. To view notes from a previous encounter, you must open a previous encounter or generate a document. Specify Cancer Medications
Access the Onc Treatment Summary template and scroll to the Oncology Medication History panel. Oncology Medication History Click the Medication name field and select an active medication from the list. Note: The list shows all active and inactive medications for a patient. You can only select active medications from the list. If you select an inactive medication you will be prompted to re-prescribe the medication in the Medication module. You must first enter a medication in the Medication module for it to appear on this list. Enter data in all the remaining fields and click Add. The system adds the record to the grid and clears the data entry fields. Document Methods of Tobacco Cessation
You can document in-depth evaluation and comprehensive information about a patient's tobacco use and methods for tobacco cessation. Open NextGen® Enterprise EHR. To open the Social History – Tobacco template, do one of the following: On the Histories template, expand the Social panel, and select Add/Edit. In the History Summary panel on the Intake template, select Social, select Tobacco/Vaping Use, and select Add/Edit. The Social History – Tobacco template opens. Expand Efforts To Quit Tobacco. From the Tobacco Cessation Information section, select the Tobacco cessation discussed link. You can also access this link from other templates. The Tobacco Cessation Discussed window opens. Tobacco Cessation Discussed To select one or multiple discussion, do one of the following: Select Single select/Close to select only one discussion item. Document Tobacco Cessation Efforts
On the Social History - Tobacco template, expand the Efforts to Quit Tobacco panel. Select from the following options for Have you ever tried to quit using tobacco: No/Never Yes Unknown Note: Selecting either No/never or Unknown means that there is no other documentation required on this panel. Enter Tobacco type. To document that the client no longer smokes, enter the Month, Day, and Year (must be four digits) next to Quit. Click the Longest tobacco free field to enter the number of days, weeks, months, or years the client quit using tobacco. Enter the Cessation method. Type the reason in the Relapse reason field. Click Add to enter the information into the grid. To document your conversation about smoking cessation methods, click the Tobacco cessation discussed check box. The Tobacco Cessation Discussed template launches.
