SOAP Template

In the SOAP template, providers can review and modify the documented reason for visits, review of systems, and vital signs. Providers can also document physical exam details and assessment plan details.

Although the provider could enter the reason for visit and HPI on the Intake template, there are options available on the SOAP tab that are not available on the Intake tab.
