Select Reasons for Visit using the Additional/Manage Link
The Additional/Manage link in the left pane of the Reason for Visit panel is used to search and select the reasons that are not available in the top ten reasons for visit.

In the Reason for Visit panel, click a reason (if available) in the grid to open the HPI template or select a reason from the left pane to open the corresponding HPI template. Note: The Reason for Visit options are dependent on the specialty selected. Enter the appropriate data in the template. Enter any comments. Click Save & Close. The data appears in a concatenated paragraph format under the History of Present Illness column in the Reason for Visit panel grid. Reason for Visit The Intake Comments link includes comments that the Intake staff might have entered in the Intake tab. If the comments are documented, the link changes color. The Comments button opens the HOPI Notes template to add any additional comments on the selected chief complaints. The dynamic HPI template opens after you select a procedure in the Reason for Visit panel. This template allows you to document the procedure details such as procedure type, procedure category, and indications. In the Reason for Visit panel, select a procedure, documented in the Procedure panel, in the grid. The dynamic HPI template opens. At the top of the HPI template, you can view the previous or next procedure available in the Reason for Visit panel. You can click the right or left arrow at the top of the template to navigate to the next or previous procedures respectively. The Reason field populates with the associated site of the procedure selected in the Reason for Visit panel. To change the reason, click the drop-down arrow and select a desired procedure from the list. Select Procedure from the left pane. A list of categories appears in the right-grid. The Reason for Visit panel on the ASC - Physician Procedure template enables the provider to document the chief complaints of the patient. Note: The Reason for Visit options are dependent on the specialty selected. Reason for Visit The procedure details documented in the Procedure panel appear in the Reason for Visit panel. If you select the procedure details in the grid, the dynamic HPI template displays to modify or add new details. If you do not want to open the dynamic HPI from the procedure in the Reason for Visit grid, you need to select the Use configured HPI from manage custom links instead of DHPI check box on the Procedure Config practice template and then link a configured HPI to the procedure in the Manage Custom Links practice template. In the Reason for Visit panel, if you select the Do not launch HPI check box and then select a reason, the system displays the selected reason in the grid without launching the HPI template. The dynamic HPI template enables you to modify the existing procedure details available in the Reason for Visit panel. The existing procedures appears in the right pane in the Reason for Visit panel. On the ASC - Physician Procedure template, click the desired row of procedure details in the Reason for Visit panel. Reason for Visit panel on ASC-Physician Procedure template The dynamic HPI template launches. In the History of Present Illness panel, click the row of procedure details to be modified. Make the necessary changes by adding new findings or removing the existing findings. To remove a selected value, click the value again. Click Close. The Reason for Visit panel is where the reason for visit, as described by the patient, is documented, along with an initial History of Present Illness (HPI). The provider should review reasons for visit, update if necessary, and complete HPI information. Reason for Visit If the Introduction field contains text, review the content and make any necessary changes. The introduction appears as the first sentence of the visit note, and it can be edited for the individual patient. A typical introduction might contain the patient's age, gender, and the reason for visit. If the introduction field is blank, compose an introduction for the patient. If checked, click to clear the Do not Launch HPI check box. In the Reason for Visit data grid, click one of the reasons for visit entered previously to open the HPI template to add additional details. You can document a patient's reasons for visit. On the Reason for Visit panel, you can document the common reasons for a patient visit. You can view the configurable specialty-specific reasons for visit list on the left pane. When you select a reason for visit that is already selected in the current encounter, a message appears highlighting the duplication. You can retain or remove the duplicate reason for visit. You can select up to 14 History of Present Illness (HPIs) simultaneously. Additionally, you can select the Do not launch HPI check box and any HPI to open the corresponding HPI template. Note: You can configure submenu items for the reason for visit on the Framework Content system template for the specialty. Based on the configuration, on the applicable template, the submenu items appear in the Reason for Visit panel on the left pane under the respective HPI. Diagnostic studies that are added to the history of present illness in the Reason for Visit panel appear in the grid wi
