Select Reasons for Visit using the Additional/Manage Link

The Additional/Manage link in the left pane of the Reason for Visit panel is used to search and select the reasons that are not available in the top ten reasons for visit. From the ASC - Physician Procedure template, go to the Reason for Visit panel, click the Additional/Manage link in the left pane. The Reason for Visit template opens. On the left side of the template, select the applicable reason(s). The selected reason(s) appears in the fields on the right of the template. If you want to select an HPI that is not included in your specialty, select the All HPIs check box to display all the HPIs irrespective of specialty. Note: If the required reason is not listed in the template, then manually type in the reason in the first available field on the right side of the template. Click Save & Close to return to the ASC - Physician Procedures template. The selected reasons appear under the Reason for Visit column in the Reason for Visit panel. Reason for Visit Panel on the ASC - Physician Procedure Template

The Reason for Visit panel on the ASC - Physician Procedure template enables the provider to document the chief complaints of the patient. Note: The Reason for Visit options are dependent on the specialty selected. Reason for Visit The procedure details documented in the Procedure panel appear in the Reason for Visit panel. If you select the procedure details in the grid, the dynamic HPI template displays to modify or add new details. If you do not want to open the dynamic HPI from the procedure in the Reason for Visit grid, you need to select the Use configured HPI from manage custom links instead of DHPI check box on the Procedure Config practice template and then link a configured HPI to the procedure in the Manage Custom Links practice template. In the Reason for Visit panel, if you select the Do not launch HPI check box and then select a reason, the system displays the selected reason in the grid without launching the HPI template. Additional Reason for Visit Panel Features

Additional features on the Reason for Visit panel enable you to adjust how you document chief complaints and HPI information, indicate follow-up visits, review the patient's HPI history, and remove studies reviewed from the grid. Patient Reasons for Visits

You can document a patient's reasons for visit. On the Reason for Visit panel, you can document the common reasons for a patient visit. You can view the configurable specialty-specific reasons for visit list on the left pane. When you select a reason for visit that is already selected in the current encounter, a message appears highlighting the duplication. You can retain or remove the duplicate reason for visit. You can select up to 14 History of Present Illness (HPIs) simultaneously. Additionally, you can select the Do not launch HPI check box and any HPI to open the corresponding HPI template. Note: You can configure submenu items for the reason for visit on the Framework Content system template for the specialty. Based on the configuration, on the applicable template, the submenu items appear in the Reason for Visit panel on the left pane under the respective HPI. Patient's Reason for Visit

You can document a patient's reasons for visit. In the Reason for Visit panel, you can document the History of Present Illness (HPI), problems, and diagnosis history as the reason for a patient visit. When you select a reason for visit that is already selected in the current encounter, an alert appears indicating that the reason for visit is already selected. You can either enter the duplicate reason for visit, or remove the duplicate. For a reason for visit that has separate listings for different specialties, such as abrasion, if you select two HPIs from different specialties for the same reason, an alert appears. You can select up to fourteen HPIs simultaneously. Additionally, you can select the Do not launch HPI check box and can select any HPI to open the corresponding HPI template. Reason for Visit on the SOAP Template

Open the SOAP template. Do one of the following: If the Introduction field contains text, review the content and make any necessary changes. If the Introduction field is blank, compose an introduction for the patient. The introduction appears as the first sentence of the visit note, and it can be edited for the individual patient. A typical introduction might contain the patient's age, gender, and the reason for visit. If checked, clear the Do not Launch HPI check box. To add additional details to a previously entered reason for visit, click one of the reasons for visit in the data grid. The appropriate HPI template opens. To add a new reason for visit: Select a complaint from the reasons for visit in the left pane. In the Other field select a reason from the resulting list or type a reason for visit.
