Update a Health Concern Goal

You can update all the fields for a goal if the goal was created during the current encounter. You can update the following fields during subsequent encounters: Goal achieved Patient priority Provider priority Target Completion Comments Status From the navigation bar, click Care Plan. The Care Plan template appears. Navigate to the Goals, Interventions and Outcomes panel. Click the goal to be updated in the left grid. The system populates the fields to the right of the grid with the data from the selected record. Make any necessary changes to the data fields. Click Update. The system updates the goal. Add a Patient Goal to Address a Health Concern

You can add multiple goals to a single health concern. To add multiple goals, repeat this procedure for each goal. From the navigation bar, click Care Plan. The Care Plan template appears. Expand the Goals, Interventions and Outcomes panel. The Select a Health concern to document section appears with all previously added health concerns. Goals, Interventions and Outcomes panel Select a health concern from the list. The information about the selected health concern goal fields appears on the right. Goal details on the Goals, Interventions and Outcomes panel In the Goal field, type a goal description to address the health concern, or use Common Phrases. Indicate if the goal is a patient goal or a provider goal. Verify that the life cycle status of the goal is set to Active. You can also change the status. For Goal achieved, do one or more of the following: Select Yes, No, Goal not attainable, or No progress toward goal. Click the field below and then select an option from the list that a

Remove a Health Concern

You can remove a health concern at any time. If you remove a concern, all associated goals, interventions and outcomes are also removed. From the navigation bar, click Care Plan. The Care Plan template appears. Navigate to the Add/Edit Health Concerns panel. Click the health concern to be removed in the lower right grid. The system populates the fields above the grid with the data from the selected record. Click Remove. The system removes the record from the Health Concerns grid. Parent topic: Manage Health Concerns

Remove a Goal Intervention

You can remove an intervention from the current encounter without restriction. You can also remove interventions from prior encounters, but you cannot remove an intervention from an earlier encounter if outcome data has been entered for that intervention during a subsequent encounter. From the navigation bar, click Care Plan. The Care Plan template appears. Navigate to the Goals, Interventions and Outcomes panel. Click the goal in the left grid that has an intervention to be removed. The system populates the fields to the right of the grid with the data from the selected record. Remove an Intervention Outcome

You can remove an outcome only in the encounter where the latest outcome was documented for that intervention. Open the patient encounter where the latest outcome for the intervention was documented. From the navigation bar, click Care Plan. The Care Plan template appears. Navigate to the Goals, Interventions and Outcomes panel. Click the goal for which you what to remove an intervention outcome. The system populates the fields to the right of the grid with the data from the selected record. Click the Intervention tab. The full Intervention tab appears with the previously entered interventions in the grid. Select the intervention with an outcome to be removed. Click the Outcomes tab. The full Outcomes tab appears with the data for the selected intervention. Select the outcome to be removed from the grid. Click Remove. The system removes the outcome. Update a Health Concern

From the navigation bar, click Care Plan. The Care Plan template appears. Expand the Add/Edit Health Concerns panel. From the bottom-right grid, select a health concern. The information for the health concern appears in the corresponding fields. Do one or more of the following: Modify the clinical status Modify the verification status Note: If you select the Entered-in-error verification status, the associated goals, interventions, and outcomes will be removed. Add or update comments To indicate that the health concern is resolved, select the Resolved clinical status and then enter the resolved date. Click Update. Parent topic: Manage Health Concerns
