Add a Health Concern

From the navigation bar, click Care Plan. The Care Plan template appears. Expand the Add/Edit Health Concerns panel. Add/Edit Health Concerns panel From the Select Category grid, select a category. The Select Concern grid appears, and all patient records for the category appear in the grid. To view all the previous records, select Show all dates. If you selected Encounter Diagnosis, Social History, or Vital Signs category, you can view all the records or filter them by a date range. From the Select Concern grid, select a record. The data from the record appears in corresponding fields on the right. If a health concern is not listed in any of the categories, use the Other category to add the health concern. To modify clinical or verification statuses, click in the corresponding fields and then select a status. The default clinical and verification statuses are Active and Confirmed respectively. Add a Health Concern Using the Other Category

From the navigation bar, click Care Plan. The Care Plan template appears. Expand the Add/Edit Health Concerns panel. From the Select Category grid, select Other. Manually enter the health concern or use Common Phrases. Select a category. To modify clinical or verification statuses, click in the corresponding fields and then select a status. The default clinical and verification statuses are Active and Confirmed respectively. Note: If you select the Entered-in-error verification status, the associated goals, interventions, and outcomes will be removed. To update the identified date, click in the corresponding field and then select a date. The default date is today's date. To enter additional information, click Comments. To enter a primary concern for the current concern, select the primary concern in the Secondary to field. Click Add. The added health concern appears in the bottom-right grid. Remove a Health Concern

You can remove a health concern at any time. If you remove a concern, all associated goals, interventions and outcomes are also removed. From the navigation bar, click Care Plan. The Care Plan template appears. Navigate to the Add/Edit Health Concerns panel. Click the health concern to be removed in the lower right grid. The system populates the fields above the grid with the data from the selected record. Click Remove. The system removes the record from the Health Concerns grid. Parent topic: Manage Health Concerns

Update a Health Concern Goal

You can update all the fields for a goal if the goal was created during the current encounter. You can update the following fields during subsequent encounters: Goal achieved Patient priority Provider priority Target Completion Comments Status From the navigation bar, click Care Plan. The Care Plan template appears. Navigate to the Goals, Interventions and Outcomes panel. Click the goal to be updated in the left grid. The system populates the fields to the right of the grid with the data from the selected record. Make any necessary changes to the data fields. Click Update. The system updates the goal. Create a Care Plan

Before you begin Make sure that the existing care plans for the patient are in Completed status. From the navigation, click Care Plan. The Care Plan template appears. Care Plan template Click Create a New Care Plan. All the panels gets activated. Activated panels on the Care Plan template Select a status, and an intent. The default values are Active, and Plan respectively. Add a health concern. Add goals to address health concern. Add interventions to achieve the goals. Record functional and cognitive assessments. Click Save and Close. Note: To edit a completed care plan, you must update the status of the current plan to Entered-in-Error, which removes the current care plan's data and redirects you to the previous care plan. Add a Patient Goal to Address a Health Concern

You can add multiple goals to a single health concern. To add multiple goals, repeat this procedure for each goal. From the navigation bar, click Care Plan. The Care Plan template appears. Expand the Goals, Interventions and Outcomes panel. The Select a Health concern to document section appears with all previously added health concerns. Goals, Interventions and Outcomes panel Select a health concern from the list. The information about the selected health concern goal fields appears on the right. Goal details on the Goals, Interventions and Outcomes panel In the Goal field, type a goal description to address the health concern, or use Common Phrases. Indicate if the goal is a patient goal or a provider goal. Verify that the life cycle status of the goal is set to Active. You can also change the status.
