Update a Health Concern

From the navigation bar, click Care Plan. The Care Plan template appears. Expand the Add/Edit Health Concerns panel. From the bottom-right grid, select a health concern. The information for the health concern appears in the corresponding fields. Do one or more of the following: Modify the clinical status Modify the verification status Note: If you select the Entered-in-error verification status, the associated goals, interventions, and outcomes will be removed. Add or update comments To indicate that the health concern is resolved, select the Resolved clinical status and then enter the resolved date. Click Update. Parent topic: Manage Health Concerns

Add a Health Concern Using the Other Category

From the navigation bar, click Care Plan. The Care Plan template appears. Expand the Add/Edit Health Concerns panel. From the Select Category grid, select Other. Manually enter the health concern or use Common Phrases. Select a category. To modify clinical or verification statuses, click in the corresponding fields and then select a status. The default clinical and verification statuses are Active and Confirmed respectively. Note: If you select the Entered-in-error verification status, the associated goals, interventions, and outcomes will be removed. To update the identified date, click in the corresponding field and then select a date. The default date is today's date. To enter additional information, click Comments. To enter a primary concern for the current concern, select the primary concern in the Secondary to field. Click Add. The added health concern appears in the bottom-right grid. From the navigation bar, click Care Plan. The Care Plan template appears. Expand the Add/Edit Health Concerns panel. Add/Edit Health Concerns panel From the Select Category grid, select a category. The Select Concern grid appears, and all patient records for the category appear in the grid. To view all the previous records, select Show all dates. If you selected Encounter Diagnosis, Social History, or Vital Signs category, you can view all the records or filter them by a date range. From the Select Concern grid, select a record. The data from the record appears in corresponding fields on the right. If a health concern is not listed in any of the categories, use the Other category to add the health concern. To modify clinical or verification statuses, click in the corresponding fields and then select a status. The default clinical and verification statuses are Active and Confirmed respectively. Remove a Health Concern Goal

You can only remove a health concern goal from the encounter where the latest intervention or outcome was recorded. When you remove a goal, the system removes all associated interventions and outcomes. Open the patient encounter where the latest intervention or outcome for the goal to be removed was recorded. From the navigation bar, click Care Plan. The Care Plan template appears. Navigate to the Goals, Interventions and Outcomes panel. Click the goal to be updated in the left grid. The system populates the fields to the right of the grid with the data from the selected record. Click Remove. The system removes the goal and all related interventions and outcomes. Manage Health Concerns

The first step in managing patient care is to identify and add the patient health concerns that you, as the provider, would like to address using a care plan. Prior to using the template, you must have entered health care concerns for the patient, either during the current encounter or during prior encounters. Allergies, smoking status, alcohol or drug use, family history, vital signs, and so on would appear as health concern categories on the Care Plan template. Update a Health Concern Goal

You can update all the fields for a goal if the goal was created during the current encounter. You can update the following fields during subsequent encounters: Goal achieved Patient priority Provider priority Target Completion Comments Status From the navigation bar, click Care Plan. The Care Plan template appears. Navigate to the Goals, Interventions and Outcomes panel. Click the goal to be updated in the left grid. The system populates the fields to the right of the grid with the data from the selected record. Make any necessary changes to the data fields. Click Update. The system updates the goal.
