Add a Patient Goal to Address a Health Concern

You can add multiple goals to a single health concern. To add multiple goals, repeat this procedure for each goal.
  1. From the navigation bar, click Care Plan.
    The Care Plan template appears.
  2. Expand the Goals, Interventions and Outcomes panel.
    The Select a Health concern to document section appears with all previously added health concerns.

  3. Select a health concern from the list.
    The information about the selected health concern goal fields appears on the right.

  4. In the Goal field, type a goal description to address the health concern, or use Common Phrases.
  5. Indicate if the goal is a patient goal or a provider goal.
  6. Verify that the life cycle status of the goal is set to Active. You can also change the status.
  7. For Goal achieved, do one or more of the following:
    • Select Yes, No, Goal not attainable, or No progress toward goal.
    • Click the field below and then select an option from the list that appears.
    • To manually enter a description, click the blank option, and type the description.
  8. Select the patient's priority and the provider's priority.
  9. To select patient's starting and completion date for the goal, click the Target start and Target completion fields and then select a date.
  10. To document more information, click the Comments field.
  11. Click Add.
    The patient goal for addressing a health concern has been added.
